My community of practice comprises healthcare professionals from various disciplines and backgrounds, including physicians, nurses, nurse practitioners, social workers, physical therapists, dietitians, and mental health specialists. We work together in various settings, including hospitals, primary care practices, long-term care facilities, public health departments, and community clinics.

The determinants of health in our community include access to health care services, access to healthy and affordable food, physical environment, social and economic conditions, education, and access to health information. As they affect the accessibility and caliber of healthcare services, a person's capacity to control their health, and the resources at their disposal, these variables greatly impact how they feel. In order to get treatment, early intervention, and preventative care, a community's access to health care services is essential. Access to affordable and wholesome food is also crucial since it lowers the chance of developing chronic diseases and enhances general health outcomes. It is impossible to overestimate the significance of these health factors in our community because they are all interconnected and significantly influence our welfare.

The most prevalent health problems in our community and our practice include obesity, diabetes, hypertension, cardiovascular disease, mental health disorders, and substance abuse. These health issues are frequently linked to socioeconomic determinants of health, such as lack of education, unemployment, substandard housing, poverty, and access to healthcare (Singh et al., 2017). Our office offers financial aid programs, screening services, and health education initiatives to assist in resolving these concerns. We also work with neighborhood health agencies and groups to provide our community with more health services. We have also put in place several efforts to improve access to care, such as longer hours, telehealth options, and improved patient navigation. We also collaborate with local partners to spread knowledge of good nutrition, exercise, and mental health through public health campaigns and neighborhood gatherings.

For this project, I have chosen to focus on a population at risk that is particularly vulnerable to poor health outcomes: the elderly population. Chronic illnesses, including diabetes, hypertension, heart disease, functional decline, and mental health disorders like depression and anxiety, are this demographic's most common health issues. Many socioeconomic determinants of health, including inadequate access to healthcare services, poverty, and a lack of social support, also affect the health of the aged population (Gardiner et al., 2018). The development of methods to deal with these issues is necessary since they can greatly exacerbate the poor health outcomes in this population.
