           During the 90’s, several pharmaceutical companies assured that prescribing opioid pain killers would not result in an addiction. This caused a misuse of this kind of medication even for minor pains easily treatable with non-pharmaceutical measures or non-opioid substances. It is tempting the almost immediate relief and other tensions that these substances cause. Of course, with this misuse, the overdose rate increase as well and today is one of the most concerning topics in health care with consequences catastrophic including neonatal abstinence syndrome, spread of infectious diseases, etc. Despite all efforts made by U.S. Department of Health and Human Services (HHS) among others institutions, this crisis is far to be resolved (Opioid Overdose Crisis, 2021). As a nurse in a rehabilitation center, almost daily I face that type of patient who shows no signs of pain; they just sit down to watch the clock and demand their painkiller very urgently. They don’t even care for the prognosis, other health issues, etc.

           At this point, to use the unlimited resources that multimedia gives us to combat this problem is highly advantageous. It can be used to communicate in real-time and on-the-go between professionals, patients, and general population. Online learning and research to information about certain topic is common these days. It is imperative to educate about the best places to find information, and if possible, to keep reliable sources up to date (Giustini et al., 2018). Is well known that media can promote health on the form of public education or social marketing, but also can be used as a tool for policy change. Media advocacy leads to political action, address a power gap, and provide the framework for the needed change. It can be used universally on any health problem taking into account the particularity of each situation (Mason et al., 2015).

           Results imperative the sustained application of certain strategies designed to alleviate this problem. Among those we found: fighting prescription fraud, facilitation of overdose treatment, ensuring proper disposal, increase adherence to evidence-based prescribing practices, and education campaigns about drug abuse (American Health Policy Institute, 2019). Yet all these efforts and resources need to be known. communication is a powerful weapon in this fight and is where social media plays a crucial role.

Abuse of Prescriptive Opioid Drugs

It is far-fetching to state the three groups of medications that are prone to abuse as they are used for medication in the health care system; prescription opioids, stimulants, and CNS depressants. It is undeniable that the abuse of prescription opioids has emerged to be a serious worldwide concern that has impacted various sectors of individuals' lives, such as economically, socially, health-wise, and other associations. A recent study coined that approximately 2.1 million individuals have been affected by this ordeal in the United States, while at least 26.4 million partakers have been impacted globally (Sebunia & Fearon, 2017).

There are assortments of variables that have been associated with an increase in this health menace. Firstly, there has been a reported increase in the stats of prescriptions of these medications to the public sector, making them more accessible and easily available. Moreover, it is a common observation that these drugs have become a norm in society and culturally acceptable, hence lacking the customary and moral checks that used to curtail the rampant use of the same (Cunningham, 2017). Moreover, there have been increased and enticing methodologies associated with pharmaceutical advertisements on opioid medications, giving the public the sole illusion of prescription opioids as the main and most effective treatment plan in managing the elaborated health manifestations. These factors have enabled the marked and extensive environmental availability of these medically administered medications.

The media can be used in the following ways to manage the adverse use of prescription opioids. First, this is the best means to reach people from all regions of the region. The media can teach, educate and sensitize on the harmful effects of opioid abuse. Moreover, the media can emphasize the role of the federal government, parents, and the community in preventing this health threat.

Health Care Policy

I consider that the problem that is giving the world health system the most headaches is the Covid-19 Pandemic, causing great wear and tear on people's health, on the hospital system and on health personnel, mainly nurses. I think that the appearance of this virus has brought very high consequences from the physical and mental point of view in all people and has brought a high economic wear. This problem has separated families, decreased them due to the number of deaths within the same family, a considerable increase in stress has occurred in people because it is a new situation. I have worked on Covid-19 floors in the hospital and in my opinion, it is a tiring job for my nurse and especially for patients, because as a nurse I see how some recover, but I have seen how other patients degrade within the same day or from one day to the next, at an accelerated speed.

 I consider that social networks are a useful instrument that helps and / or contributes in the transmission of information, at the level of person, population and country, of the different issues that exist. Social media has played a very important role during this pandemic. Those responsible for the CDC and the health system, through social networks, have disclosed the benefits of vaccination against Covid. Through the networks they have exposed all the information necessary to convince a large majority of the population to get vaccinated. Another smaller group has used social media to combat such a vaccination program, exposing what are considered negative effects. The authorities have managed to establish a central and well-defined objective regarding this issue. We have also, as nurses, used social networks to promote the vaccination plan, which would help us significantly to take a break if the incidence of the percentage of cases decreases because on many occasions we have worked up to 5 days.

Health Care Policy

           A health policy that needs to be highlight is obesity. Obesity is a growing problem in all of our communities and increasing the ability for adults to lead a healthy and fruitful life. Obesity leads to several health challenges such as heart disease, diabetes, and can even increase the incidence of cancer. Women that are overweight have a higher incidence of uterine cancer that requires a hysterectomy or breast cancer requiring mastectomy. Educating this population through health policies and advocacy can help decrease the epidemic of obesity. The difficult part of the adult obesity issue is that until an individual desires a need to change, they will not. This is where social media can make a difference. 

           Incorporating awareness and education into the changes in the community may provide some of the people that face this battle the information as to how to make the change. APNs can use social media to spread knowledge and information by providing the ability for individuals to contact an authority on nutrition and exercise. More importantly, media outlets or organizations do not see themselves as a part of, or contributing to the public health system. As this chapter discusses; however, the media plays a number of roles in educating the public about health issues and has a responsibility to report accurate health and science information to the public.

As social media has evolved, APNs have established a way to tap into more in the community to get their message across. Nurses have created Facebook pages and networking sites to educate the community on various topics. Media-sharing sites, such as YouTube, offer a large selection of social media tools that are optimized for viewing, sharing, and embedding digital media content on the Web to attract attention. They also provide features that are typically found on other types of social media sites, such as profiles, connections, comments, and private messaging.

Alcohol use disorder

The group of alcohol use disorders can be characterized by compulsiveness and loss of control over the amount of alcohol ingested and represents one of the mental disorders with the  highest prevalence in countries with high and moderate economic income (Carvalho et al., 2019). This aspect associated with high mortality and morbidity make it a public health problem that is difficult to control despite the large number of treatments and interventions available.

 

Within the possible interventions to attack the problem emphasis is placed on the primary care screening and all types of brief interventions, but despite that there is no significant reduction in its prevalence. In 2016 there was a meeting of the International Network on Brief Interventions for Alcohol & other drugs and they found that one of the effectiveness gaps is because screening is only effective in alcoholism that is self-reported but has no greater effect on the referral on another member of the family or community (Glass et al., 2017). One of the points highlighted at that meeting is the reconsideration of health personnel’s pre-existent concepts about alcohol consumption that could affect health care. In this sense I think it is important to highlight that there is a type of alcohol addict that is hardly recognized because it can maintain a certain degree of functionality with some kind of professional and family success (high-functioning  addict). This is the type of person who masks their exaggerated consumption of alcohol with the excuse of releasing the tensions generated in their work or the celebration for the successes achieved. It has become a celebrated and sometimes envied stereotype in social media or television programs, creating a charming idea that is difficult to link with a true addiction.

 

In this sense, I am convinced that we need more policies about  the content of Tv shows, movies, commercials and other types of advertising that could incite consumption and achieve awareness campaigns about the different faces that unhealthy alcohol consumpt

           Undoubtedly, a public health problem that should be highlighted today in health policy is Covid 19, this lethal virus has weakened the health of our population. Not only because of what the virus itself is, in terms of symptoms or treatment, this virus has affected many families, both physically, psychologically and socially. A virus of such magnitude stopped the world and as for the health of our population it’s in our hands to mitigate its effects, since it has become a pandemic, an endemic virus and we have to learn to live with this.

           Social media is a valuable tool influencing health policy makers. Social networks promote communication between people and help promote topics of interest, exchange knowledge and transmit ideas (Mason, Gardner, Hopkins & O’Grady, 2016). In the case of mass immunization, for example, it helps to convey messages regarding its safety, efficacy, and the need for vaccination.

           The usefulness of social media is given, for example, once the analysis of a health policy is completed, which helps to solve or mitigate a health problem, it's essential to develop a plan that serves to identify appropriate tasks and strategies to achieve the purposes of policy. Develop and implement a political strategy to solve a precise problem with a strict message, a common objective that aligns and an impartial audience that is well defined. Messaging is crucial to developing a political strategy. Nurses must be able to communicate with policy makers, other healthcare leaders, and the public, and can often use social media to send messages, to advise on institutional and public policies (Mason et al., 2016).

           One of the most important functions of the media is to convey problems to the public and to policy makers. The way in which the media covers debates on various topics helps to identify issues of great importance. An example of this is the media coverage of patients who lost their lives because they did not have health insurance to pay for treatments such as cancer and other serious conditions, this inclusion of the media facilitated support to carry out a health care reform and the approval of Affordable Care Act (Mason et al., 2016).
