Health care justice at Cuyahoga County Corrections starts with the organizational readiness to promote change.
Understanding the roles and the scope of practice of individual team members, self-evaluation, and awareness of team members strengths and weakness must be accomplished with the support of management.
It is crucial for the CCC to learn how to collaboratively interact in an ever-changing environment that promotes the health, wellness and safety of our patient population.

TEAM OF SPOTTERS – this team will be located in Sally Port where all inmates arrive.
Vital signs of all inmates that enter into the CCC will be measured during the classification prescreening perform by the corrections officers.
Measuring and recording inmates vital signs accurately is important as this gives an indication of the patient's physiological state.
Vital signs and answers gathered will prompt the priority of care and will allow a simple but needed health assessment. (Recorded on health card for intake assessment team.)

d. Diabetic (Accu-check)
e. Cancer (medications)
f. Kidney failure (facility information if dialyzed)
g. ETOH – (last use/ how much)

TEAMS
INTAKE ASSESSMENT TEAM- Assess individuals according to the severity of health care needs. All incoming medication needs to be evaluated by nursing staff to determine prioritization of medical and mental heath needs
DISPENSARY TEAM - Assess inmates who are coming into Sally Port, give urgent calls to health assessment runners (paramedics, nurses) to assess for health services and the need to go to the dispensary.
MEDICAL EMERGENCY TEAM- All qualified health staff must be on alert.
TB/ Kite TEAM- Interpret all TB readings and evaluate kites for services

CART TEAM- focus on safe and efficient deliver of medications (carry kite, report any known immediate health concerns)
MEDICAL FLOOR NURSE TEAM – Assess medical, prescription and equipment needs (if the inmate has equipment during initial assessment must have during LOS)
MEDICATION RECORDER TEAM – Recording all new medication orders and retrieval from the pharmacy (assist cart nurses for medication retrieval if the cart is of late return)
WOUND TEAM – All wound care and documentation
DIABETIC TEAM – Assess levels and health concerns

Mental Health Team
All Mental Health issues will be assessed in the Mental Health area
All intakes which involve Mental Health will be assessed by a Mental Health nurse.
New Mental Health commits
Inmates in restraint chairs (initial placement and removal)
All Mental Health inmates will be initially assessed by a Mental Health nurse if problems arise while on the unit.
Mental Health is to contact medical only if acute declining health.
No inmate will be brought to medical who has been pepper sprayed those inmates will be assessed and restraints will be checked in the restraint room.
This will help to decrease confusion, hostility, and conflict
