Deployment Flowchart

Patient Background

Educated and active female patient in her mid-forties that is married and has three children.

Exercises

Balanced diet

Non-smoker

Occasional alcohol

History of asthma requiring occasional therapy.

History

Mild intermittent right lower quadrant pain that woke up patient

Low grade fever

Applied heating pad

Pain and fever subsides

Pain and fever returns a few hours later

Patient presents at doctor’s office

Nurse Assessment

Patient taken directly to exam room upon arrival

Vital signs and assessments are completed

Elevated temperature- 102 degrees

Rapid heart rate

Sweating

Holding right lower quadrant

Requests provider evaluation immediately.

Physician Assessment

Physician determines that based on patient’s symptoms she should be transferred to emergency room for further evaluation.

Emergency Room

Oxygen is applied

Patient is NPO

IV is started

Patient receives fluids, pain and anti-nausea medication

Exam completed- diagnosis appendicitis

Recommended immediate surgery

Post Surgery

Surgery successful

Patient transferred to med surg floor

Nurse identifies non-productive cough and crackles in left lower lung

Consultation with resident chest x-ray ordered

Patient diagnosed with lower lobe pneumonia

Antibiotics and medications started

Additional 2 days of hospital care needed

Discharge

Patient PHI sent to patient’s physician

Appointment scheduled with surgeon

Appointment scheduled pulmonary specialist

Appointment scheduled with primary care provider

Patient attends post op meeting

Recommendations made

Patient Recovery

Patient receives in home nursing care for pneumonia

Patient condition slowly recovers

Patient experiences fatigue but improves

Patient sees primary care physician for follow-up
