Evaluation of Pediatric Exercise ‘‘Vital Sign’’in Electronic Medical Record from Sports Medicine Clinic. Anything related to medical care such as diagnosis, treatments, prescriptions, doctors names, hospital info, dates, procedures, health history, family history, social history, etc). Basically, anything that would be in your medical record for any health care provider (Young & Valasek, 2018). Personally identifying information (name address phone DOB insurance info, etc) is also protected but is classified differently under HIPAA and consequences of violations are a little different. 1. The Electronic Medical Record (EMR) The Electronic Medical Record (EMR) is the presentation of health records in a digital form unlike previously when the health records were presented in paper form. (Young & Valasek, 2018). The EMRs have gone, in short order, from novelty to necessity. Propelled in no small part by federal edict, they are now as common as a stethoscope, the hope being that their use will lead in time to better, less expensive care (Scott et al., 2018). In other words, EMS simplifies the entire process. No longer is it a matter of rooting through a bulging envelope and finding the proper paperwork 4. Privacy Pledge I am pleased to inform you that the Three Mountains Regional Hospital respects the privacy of all our clients and the hospital has drafted this privacy pledge statement to illustrate its determination and commitment to patients’ privacy and confidentiality protection (Young & Valasek, 2018 )This privacy policy statement is meant to define for you the user of the hospitals website, mobile phones, online accounts and any related website. We, therefore, request you to read this privacy policy statement very carefully before using any of our website or online resources. Pledge: We, therefore, pledge to continue protecting the privacy of all our clients and ensuring their information remain confidential through appropriate use of patients’ personal information based on the above commitments and also in strict adherence to HIPPA laws applicable to us as the healthcare service providers. Data are safeguarded to prevent any unauthorized access. HIPAA standards protect the privacy of the patient by restricting certain uses and disclosure of the health information (Scott et al., 2018). It also allows the patient to get access to their data anytime. HIPAA confirm that if there is any breach of the health information, the entity must inform it to the affected patient. 3. Release of health information The patients must always make a request to review either their health records copies or sections (Young & Valasek, 2018). This request needs to be made in a written and formal form. After the request is made is when the physician is allowed to send it to the patient. Nonetheless, patient information cannot be sent to him/her without his/her consent (Young & Valasek, 2018). The HIPAA Security rules are only concerned about PHI that is digital or electronic in form (ePHI) and resides on digital media. So in a case where paper medical records are placed into a dumpster, the violation is only related to the privacy rules, not the security rules since the data was not digital in form. Now every aspect of a patient’s medical history is spelled out for doctors and nurses, and just a mouse click or two away (Young & Valasek, 2018). Important information is far less likely to fall through the cracks, and the possibility of errors in caregiving is minimized. According to the Joint Commission, the health records needs to contain the patient’s medical history and main medical patient’s data such as: Demographics Vital signs Diagnoses Medications The Joint Commission also wants all the EMS records to be completed within 1 day/24 hours of the encounter. The time they are entered must also be entered, dated and signed. 2. HIPAA/Confidentiality The Health Insurance Portability and Accountability (HIPAA) is a US health law that was enacted made to outline the privacy standards to safeguard the EMS records, and provide clear guidelines on how doctors, nurses and care providers should handle health records (Scott et al., 2018). Privacy of healthcare information means the patients are supposed to be left alone to provide the information they want to provide without being compelled to do so. This privacy covers all the forms of PHI which include paper records, electronic health information, films, and even the spoken information.
