It also helps identify the identified systematic variations on their specified types of patterns regarding the occurrence. It even applies the identified resulting knowledge to advance and implement the most appropriate policies and even actions to enhance the identified health and well-being regarding the given population. The following research paper offers an executive summary on the population health management strategies being developed for Cardiovascular patients of all ages for improved population health outcomes.

Possible techniques for the grouping of the population to enhance care entail age and gender, illness, or condition, social and demographic elements, behavior, and the identified risk stratification on a single or even more of these types of variables. The most often utilized strategies are by age as well as illness. The focus, in this case, is on the segmentation of the patient population via the utility of risk associated stratification, which is a vital step in the development of the most appropriate type of population health strategy. Primary care physicians provide care to patients and are comprehensive, coordinated, highly accessible, and focused on the safety and quality of the service delivered. They aim to ensure that service delivery is in the correct order and for maximum outcomes to help patients live a healthy life (Weinstein et al., 2020). They are usually comprised of various providers in distinct disciplines to offer comprehensive and personalized care. This is to ensure that personalized and highly skilled service professionals are involved in the delivery of care. This is vital in dealing with the different aspects of cardiovascular illnesses among different types of patients for success in delivering services to the identified patients. The foundation regarding the primary care physician is the relationship between the patient and their physician. This physician usually leads one's health and is involved in the promotion of wellness. In addition, they assist patients in the prevention of illnesses such as cardiovascular disease.

Focusing solely on the sick patients and the patients associated with having existing elements will be inappropriate in the control of the healthcare costs on a long-term basis. There is the need to use the types of interventions that are highly focused on the short-term goals and the long-term basis for maximum outcomes and efficiency in the whole process. The most significant driver regarding the escalation of the identified healthcare costs is an expensive treatment for chronic illnesses such as cardiovascular and high blood pressure; it is vital to understand that a highly preventive type of healthcare is essential (Dames et al., 2020). This is an effective strategy in the public health management of cardiovascular illnesses; it helps focus on what can be done in practice to ensure that future efficiency is achieved and no negative implications occur. The identified prevention of cardiovascular disease is highly vital in the reduction of the increasing healthcare-associated costs. There are sources for this disease and evidence-based preventive recommendations that healthcare-associated providers can access globally (Dames et al., 2020).

Evidence usually highly suggests that the individuals living with disadvantaged circumstances tend to be less healthy than their highly advantaged counterparts. Moreover, it is vital to note that the individuals who grew up in poverty cases and the substandard types of housing are associated with higher mortality.
