Community health assessment needs

Healthcare needs to incorporate crucial measures that shape change while ensuring a focus on specific measures that present a broader basis for change and an emphasis on healthcare quality. The health needs, in this case, emphasize cardiovascular diseases and measures that can be investigated to control the spread of the disease. For example, to reduce cardiovascular diseases in Lake County, the community can offer affordable healthy foods, exercise classes, and education. In addition, decrease access to tobacco and drugs, especially to minors (Lakecountyhealth.gov, 2021).

Controlling comorbidities

The spread of cardiovascular diseases is necessitated by the lack or presence of comorbidities such as diabetes, high blood pressure, high cholesterol, obesity, smoking, and hypertension. Therefore, it is imperative to ensure that these diseases are controlled to limit the likelihood of adverse outcomes in controlling cardiovascular diseases.

Heart disease is the leading cause of death among patients with type 2 diabetes (Murphy & Goldman, 2020). Individual education is essential for minimizing cardiovascular disease. Diabetes patients must be informed of the effects of the disease on their bodies. Sugar is used as an energy source by tissues, stored in the liver (Murphy & Goldman, 2020). Sugar can linger in a diabetic's bloodstream and seep out of the liver, causing damage to the blood vessels and nerves (Murphy & Goldman, 2020).

High blood pressure is a common cause of heart disease and stroke (Lake County, 2021). High blood pressure puts a load on the heart and affects the blood vessels (Murphy & Goldman, 2020). People with high blood pressure should eat a nutritious diet, exercise regularly, and take their medications as prescribed (Murphy & Goldman, 2020).

High cholesterol raises the risk of heart disease and stroke (Lake County, 2021). This can result in blockages and a heart attack (Murphy & Goldman, 2020). The most significant factor influencing one's cholesterol is genetics (Murphy & Goldman, 2020). However, it is critical to maintaining a healthy lifestyle and workout routine (Murphy & Goldman, 2020).

Obesity has a strong influence on the above risk factors (Murphy & Goldman, 2020). Therefore, an effective way to manage weight is to work with a nutritionist and exercise regularly (Murphy & Goldman, 2020). In addition, smoking causes a buildup of plaque in one's arteries. Plaque can cause one to have a heart attack or stroke. Therefore, individuals should be educated on the risk of smoking and offered ways to quit smoking.

The demographic data provide a solid foundation for identifying the underlying difficulties in the social setting. The age distribution is an essential component in improving efficiency and development. Older patients have a higher risk of dying. This means that keeping track of a person's age is critical to reaching a higher level of transformation and a dedication to personal accomplishment. Most cardiovascular diseases can be avoided by avoiding risk factors like cigarette use, a poor diet, obesity, a lack of physical activity, and the harmful use of alcohol and other substances. People who have cardiovascular disease or are at high risk for it require early detection and treatment.

Wellness programs

Wellness initiatives are also an essential part of changing people's lifestyles. CVD patients need to change their lifestyles because they have a limited range of activities they can do independently. In addition, there are significant restrictions that must be adequately assessed to achieve the required level of care (Knowles et al., 2019). Dietary restrictions, exercise, and stress reduction provide a solid foundation on which patients can enhance their quality of life.
