My sleep schedule is a little off. I work the night shift schedule about 4xs/week so I sleep during the day. My alarm usually wake me up, so I wake up in time. The following day when I’m off, I would sleep until 3pm so that I will be able to sleep easily at night. My 2 kids usually tire me out by then so it all works out. |

I’m a single mom so I don’t want to bother/inconvenient anyone to babysit my kids. My brother and aunt already help me with the kids while I am at work. Application of Health Promotion Assessment (PAPER #1)
Based on the assessment, diverse results show some abnormalities. For example, based on the diet intake of the patients, the diet is highly organed and balanced with the consumption of whole foods, nuts, fruits, and vegetables which possess multiple health and well-being benefits. Moreover, the patient requires more education on nutrition issues. Based on the client's curet food intake, the results indicated a high level of knowledge on eating healthy through combining different and essential food items during breakfast, lunch, and supper. Equally, despite the client being pre-diabetic, there is a lack of preventive measures and healthy living, which could increase the risk of the disorder's advancement (Genewick, 2020). For instance, engaging in different physical activities, reducing a sedentary lifestyle, and increasing physical activity are essential in healthy living.
The assessment reveals various normal activities and information based on the report results. For instance, the participant nature of commitment to different daily chores creates barriers to engaging in physical activities. Attending work and having children at home enhance participant's tight schedule which prevents the participant from engaging in physical activities that improve her health by being pre-diabetic.
Style of Learning the Participant Prefers
The participant prefers learning through visuals due to a lack of impairments. Visual learning comprises PowerPoints, posters, pamphlets, brochures, and demonstrations, increasing the participant's learning and grasping of health information. Visual learning is an essential strategy in the healthcare system resulting from the increased capacity for the participants to understand the information being communicated. Education is a significant factor in improving the safety and quality of care, ensuring enhanced patient experience and outcome. Moreover, using the visual learning technique mentioned by the participant contributes to different benefits. For instance, such a learning style is essential in the healthcare system due to facilitating stimulation of the images and effects on their cognitive capabilities, increasing recalling of the learned information. The retainability of the learned information by the participant in 10/10 indicates a high level of learning efficiency. As a result, the education offered more so through the visual styles is significantly absorbed by the participant leading to an accurate understanding of health and well-being. Such participants have a vital role in increasing the efficiency of the healthcare system resulting from implementing health education taught through diverse practices and behaviors.
Long-Term SMART Goal
Specific
Changing towards eating healthily and engaging in exercise. Healthy eating involves decreasing soda intake to 3 ounces per week, increasing fruits and vegetables to five serving a day, and reducing daily sugar intake to 25 grams of added sugar. Engaging in physical activities such as evening walking and jogging for 20 minutes every Monday, Wednesday, and Saturday.
Measurable
Taking at least five servings per day from the current 2 serving per day. Reducing soda intake to 3 ounces and sugar to 20 grams. Evening walking and joggings three times per week to monitor the progress and changes in weight.
Attainable
Gradual increase in the intake of fruits and vegetables and decrease in sugar and soda intake for the three weeks.
Realistic
Healthy eating and physical exercises to loos 1 pound per week.
Time Bound
Engage in exercise three times a day for three weeks. Intake of five servings of vegetables and fruits for three weeks and reduce intake to 3 ounces for three weeks.
Short-Term SMART Goal
Increasing water intake from one glass per day to 8 glasses for one week.
Specific
Improving water intake from 1 glass to 8 per day in one week.
Measurable
The number of water glasses taken per day determines the progress in increasing the amount of water intake.
Attainable
Gradual increases in the number of glasses from 1 to 2 on Monday and four on Wednesday until 8 glasses by Friday.

Reaching 8 glasses of water intake within one week.
The participants require diverse needs and resources to accomplish and attain the short- and long-term SMART goals. For example, the client requires education and training on the myriad benefits of achieving those goals (Genewick, 2020). Besides, the participant needs a well-organized schedule for the diverse activities to attain the goals effectively. For example, the participant required adequate education and training on physical activities, reducing soda and sugar intake, and increasing the intake of fruits and vegetables in the three weeks). Educating and training participant on the benefits of attaining those goals enhances increased participation based on the participant's positive health and well-being outcomes (Yen & Leasure, 2019). Besides, the participant requires time to engage in physical activities instead of only focusing on work and family issues.
Chronic disorder such as diabetes is a significant topic from the assessment of the client based on diverse rationale. Health education should focus on educating and training the participant in preventing measures for diabetes based on pre-diabetes conditions (Yen & Leasure, 2019). Furthermore, the motivation for the topic originates from participants being at high risk of developing chronic disorders due to sedentary lifestyles without engaging in physical activities.

· Week 10 you will evaluate your teaching plan and participant's response to plan.
Weight loss refers to creating a calorie deficit through dietary changes, increased physical activity, and different nutritional interventions (Freire, 2020). Maintaining a healthy weight is essential for overall health. This is because excessive weight is associated with health complications such as heart disease, diabetes, and joint issues (Al-Nimr et al, 2020). The participant indicated that the main topic to focus on is weight loss and nutrition. The first short-term goal was to improve nutritional knowledge and healthy eating habits. Information is critical in reducing weight loss. Therefore, the participants will be given nutrition information in the first week. The information will be obtained from nutritional books, dietary guidelines and healthy eating habits. Food portion determination is essential in weight loss. The patient will be taught how to portion their food. This will be facilitated using different food measuring tools. The client will be taught how to use the measuring tools in the teaching. Similarly, mindful eating skills will be taught to allow the participant to consume foods, stopping when satiated. Also, the participant will be taught how to balance calorie content and achieve the sustainable calorie deficit required to reduce body weight.
In the second week, the goal will be to learn how to diversify food choices to achieve a balanced diet. This goal helps the participant incorporate different foods into the diet. This includes fruits as a source of vitamins and minerals (Al-Nimr et al, 2020). Moreover, the participant will be taught to incorporate whole grains such as brown rice, whole wheat and quinoa in the diet as a source of fiber promoting fullness. Hydration is an important factor in weight loss (Al-Nimr et al, 2020). Therefore, the participant will be taught the role of hydration and strategies to implement to achieve the required hydration for weight loss and overall health.
In the third week (week 10), the goal will be to learn physical activities vital to weight loss. This includes activities such as aerobic exercise for at least 150 minutes per week. Besides, the participants will be informed on how to incorporate weight training exercises at least twice every week. The long-term goal is to achieve consistent and sustainable weight loss. The intervention will be a three-week duration. The activities will be divided based on the three goals. After three weeks, the patient will be evaluated on whether the expected outcome has been achieved. This is vital in informing the strategies that will be used in helping the patient achieve the desired weight.
In the teaching, the participants prefer the use of visuals. Therefore, visuals such as educational pamphlets will be incorporated into the weight loss and nutrition discussion in order for it to be implemented it back home. During the teaching intervention, the subject will be given a chance to answer questions and seek clarification on what is unclear to her.
Nutrition and weight loss are important topics globally. Therefore, information on this topic is found in many resources. In this intervention, diverse resources will be utilized. First, education pamphlets will be of importance. The education pamphlets are vital in teaching patients about effective eating plans. Besides, they are essential in describing the different foods and the micro and macro components (Freire, 2020). In addition, books and articles published in reputable health and nutrition will be useful. The patient will be recommended to read selected articles and books. Through this, the patient will have overview information on the topic of interest and reinforce what will be learned during the teaching intervention.
At the end of the intervention, the patient will be evaluated based on several outcomes; on the first goal, the patient will be evaluated on her ability to explain nutrition and the impact of nutrition to her health (Brauer et al., 2021). The patient should be able to explain how to determine food portions while cooking and serving. Also, she should give information on the best strategies to ensure that she consumes the required food portions.
On the second teaching intervention goal, the patient is expected to share information on best practices of ensuring that the meal she consumes with her family is balanced (Freire, 2020). This incorporates information about fruits, vegetables, proteins, carbohydrates and fiber. The food should consider vital micronutrients such as minerals and vitamins (Brauer et al., 2021). The patient will also be expected to have adhered to the prepared meal plan that is customized for her.
On the third goal, the expected outcome is that the patient will strictly adhere to the physical exercise routine. This includes aerobic exercises for at least 150 minutes weekly and muscle straining at least twice a week. The patient is expected to participate in physical activity consistently without supervision. The patient is expected to achieve the desired weight on the long-term goal. The patient's weight is expected to be in balance with the body mass index through shedding excessive weight.
Before the teaching intervention on nutrition and weight loss, the patient will be asked the following questions;
1. What do you think a healthy diet consists of?
2. Do you think exercise is important? Why or Why not?
3. How long do you think you should exercise for when trying to lose weight?
4. What is the importance of practicing mindfulness?
5. Do you believe stress is a contributing factor to unhealthy eating habits?
After the three weeks of teaching intervention, the patient will be asked the following questions
1. How has your dietary intake changed since you commenced the weight loss and nutrition intervention? Are there some foods that you have reduced or increased?
2. How would you describe your calorie before and after the intervention?
3. How would describe your mood, attitude, body endurance to exercise and energy levels since you started the weight loss and nutrition intervention?
4. Do you think you can make informed decision on the choice of food you incorporate in your daily meal?
5. Do you have anything to celebrate on your weight loss journey?
