Example... PART I - Description of the problem- I have a niece, Lulu, who is having trouble reading and is beginning to feel bad about herself. She is also saying that she hates reading. She tries to pretend that she is sick to avoid going to school. She brought home a bad grade on her last assignment and came home from school crying one day because her classmates had laughed at her when she tried to read aloud. (This is a hypothetical example).

PART II- Description of the goal- I would like my niece to improve her reading skills and feel better about herself. I want her to love going to school and get a great education. I would like her to enjoy reading as much as I do.

PART III - Description of procedure for reaching the goal- (Be specific) I will work with my niece practicing reading 3 times a week for 30 minutes each time. We will play reading games that I get from her teacher or online. I will also take her to Chucky Cheese after each test she takes that shows improvement. I will also give her stickers or maybe let her choose a movie to watch. We will always take time after practicing to go outside and play her favorite game, Freeze Tag. The skills I will use are active listening, praise, affection, and bargaining. I will actively listen as she explains where she has difficulties. I will praise both her efforts and when she improves or gets something right giving stickers or saying, “wow, that was great!” etc.. I will give affection by hugging her when she is frustrated and when she does a good job. I will use bargaining when I tell her that we will go to Chucky Cheese after she gets a better grade on her next test as well as playing her favorite game after we do our work.

PART IV - Determine when the goal has been reached - I will feel like my job is accomplished when my niece’s reading skills improve and she is able to make a good grade on her next several reading tests. I would also like her to feel like reading is a positive and fun experience. This should take about nine weeks to complete.
