Answer:
Both data dictionaries and data sets can be considered as important tools, in a healthcare setting, when it comes to the collection, organization, and analysis of patient care information.
A. Data Dictionary – This usually contains information related to a variety of data elements which relate to patient care. They include:
1. The patient’s demographics: This are the patient’s age, name, gender, and race.
2. Clinical diagnosis: ICD-10-CM (diagnosis code), ICD-10-PCS (Procedure codes) etc.
3. Procedures: CPT codes
4. Medication prescribed to the patient: This includes the names of the drugs and the dosage
5. Laboratory values: This includes cholesterol levels, blood glucose levels etc.
B. Data Sets – Utilized in the reporting and collection of specific quality measures which relate to patient care, such as measurement of hospital readmission and the tracking of compliance with clinical guidelines.
1. Measure Title: The name of the quality measure being tracked (e.g., "30-day readmission rate").
2. Measure Description: A brief explanation of what the measure is tracking and why it is important.
3. Measure Population: The patient population that the measure applies to (e.g., all hospitalized patients).

Case study 1: Margaret, a registration clerk, is trying to enter the patient’s state of Arizona. Every time she starts typing it, Arkansas pop up.
Question 1: Identify the issue preventing the correct state from being entered? ( 2point)
Answer: The problem that prevent entering the correct stats she writing AR, which, which postal code for Arkansas so she need to write AZ for Arizona.
Case study 2: Margaret begins to enter the patient’s telephone number of 616-256-6767 and only gets as far 616-256-67 and the field won’t accept any more characters.
Question2: Determine the issue with the data dictionary? ( 2point)
Answer: The problem with the data dictionary is that the field only accepts 10 characters, so she must remove the dashes to be able to type all the numbers

Case Study 1 : ( 4points- 2 point for each paragraph analysis )
Questions: Identify the HIPAA Privacy Rule concept that is at issue in each paragraph of this scenario. Has community Hospital’s privacy officer correctly interpreted the privacy rule requirements in each situation? Why or why not.
.First scenario: Recently, the hospital’s emergency room staff has informed the privacy officer that patient’s family members have been bringing cameras into the emergency room to photograph treatment as it is occurring. The privacy officer instructs emergency room staff to log the names of individuals who are tasking the pictures. He then sends letters to the family members , notifying them that they have committed a HIPAA violation and their names may be turned over to the office for civil Rights for investigation.
Second scenario: Elderly patients are frequently discharge from the hospital to local nursing homes for skilled rehabilitation following orthopedic issues such as hip fractures and many types of surgeries. Staff on the units from which patients have been discharged have routinely copied the entire patient chart to accompany the patient to the nursing home, but the privacy officer informed them that this is a violation of the HIPAA privacy Rule
Answer: HIPAA Privacy Rule concept: The privacy act is still being break although they are related to the individual being treated.
. Community Hospital’s : They hospital must do as they say to tell the people filming and taking pictures they are violating the act.

Second scenario answer: HIPAA Privacy Rule concept: The chart is needed to be safe at the original hospital.
.community hospital: the hospital needs to look into who is copying the charts and write or charge them with breaking the rules

Correct the chart can only be obtained with a signed consent from the patient and needs to be okayed by the head of the hospital.
