Community Teaching Work Plan Proposal
Planning and Topic
Directions: Develop an educational series proposal for your community using one of the following four topics:
1. Bioterrorism/Disaster
2. Environmental Issues
3. Primary Prevention/Health Promotion
4. Secondary Prevention/Screenings for a Vulnerable Population
Identify which one of these is relevant to your topic
Planning Before Teaching:
|
Name and Credentials of Teacher: Your name |
|
|
Estimated Time Teaching Will Last: How long will this presentation be? |
Location of Teaching: Where will you be doing this? Identify place (community center, hospital, etc.) If you are doing face to face presentation, identify how you will address social distancing & masking, etc. If you are going to do this presentation virtually, describe how this will work |
|
Supplies, Material, Equipment Needed: Handouts, LCD projector, etc. |
Estimated Cost: Do you need to buy anything? (pay for printing, etc.) |
|
Community and Target Aggregate: Who are you teaching? Be as specific as possible (7th grade students of X Middle School, entrance screening workers at X Hospital, etc.) |
|
|
Topic: What are you teaching? Importance of self-care for staff working COVID unit of X Hospital, diabetes management for teens at X Community Center, etc. |
Identification of Focus for Community Teaching (Topic Selection):
How did you choose this topic? Did someone ask? Did you/others observe a need? Has there been a change in process/recommendation?
1-2 paragraphs for each of these
Epidemiological Rationale for Topic (Statistics Related to Topic):
What does the literature say about this topic & why it is important?
Teaching Plan Criteria
Your teaching plan will be graded based on its effectiveness and relevance to the population selected. This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.
Nursing Diagnosis:
You don’t need an actual nursing diagnosis here, but do identify a problem statement
Readiness for Learning: Identify the factors that would indicate the readiness to learn for the target aggregate. Include emotional and experiential readiness to learn.
How do you know the people you want to train are open to learning? (they requested it, they responded to a survey, etc.)
Learning Theory to Be Utilized: Explain how the theory will be applied.
Pick a learning theory to use. Who are you teaching? What do we know about teaching that age group? (Adults- have previous life experience, learn by doing----Children- may not know anything about topic, need short, attention catching information, etc.)
Goal: Healthy People 2020 (HP2020) objective(s) utilized as the goal for the teaching. Include the appropriate objective number and rationale for using the selected HP2020 objective (use at least one objective from one of the 24 focus areas). If an HP2020 objective does not support your teaching, explain how your teaching applies to one of the two overarching HP2020 goals.
Look at the Healthy People 2020 objectives. Pick one that is related to your topic (Preparedness, Diabetes, Mental Health & Mental Disorders, etc.)
How Does This HP2020 Objective Relate to Alma Ata’s Health for All Global Initiatives
Look up Alma Ata’s Global Initiatives. How does the HP 2020 objective related to that?
Develop Behavioral Objectives (Including Domains), Content, and Strategies/Methods:
|
Behavioral Objective and Domain Example – Third-grade students will name one healthy food choice in each of the five food groups by the end of the presentation. (Cognitive Domain)
Need at least 3-4 objectives Learner based Measurable & specific (discuss, identify, demonstrate, etc.)
Use these examples as a guide |
Content (be specific) Example – The Food Pyramid has five food groups which are…. Healthy foods from each group are…. Unhealthy foods containing a lot of sugar or fat are….
Use these examples as a guide
|
Strategies/Methods (label and describe) Example – Interactive poster presentation of the Food Pyramid. After an explanation of the poster and each food category, allow students to place pictures of foods on the correct spot on the pyramid. Also, have the class analyze what a child had for lunch by putting names of foods on the poster and discussing what food group still needs to be eaten throughout day.
How are you going to teach? PowerPoint presentation, scenarios, discussion, demonstration, etc. |

Learners will demonstrate how to check their blood pressure with a wrist cuff
etc.

Planned Evaluation of Goal: Describe how and when you could evaluate the overall effectiveness of your teaching plan.
You need to evaluate at the end of the program
Did learners successfully complete the objectives?
Planned Evaluation of Lesson and Teacher (Process Evaluation):
How would you have the learners evaluate the program & you as teacher?
Written evaluation form, verbal feedback, etc.
Barriers: What are potential barriers that may arise during teaching and how will those be handled?
What problems might come up? Noisy location, high traffic location, lack of learner interest, needed equipment not available or not working properly, etc.
Therapeutic Communication
4.2 Communicate therapeutically with patients.
How will you begin your presentation and capture the interest of your audience? Describe the type of activity will you use with your audience to exhibit active listening? Describe how you applied active listening in tailoring your presentation to your audience? How will you conclude your presentation? What nonverbal communication techniques will you employ?
How are you going to catch their attention? Telling brief story, giving True/False questions, catchy graphics, etc. If participants seem uninterested, how can you modify your teaching to improve that?
This assignment does need to go through LopesWrite
The scores may be very high because it is a template
Don’t stress about this (as long as you are using your own words, etc.)
I will review the report also
Once you have completed this After completing the teaching proposal, review the teaching plan proposal with a community health and public health provider in your local community. You can do this virtually (phone, Skype, etc.). Ask for their feedback about what will work, what could be improved, etc.
Complete the "Community Teaching Experience" form with the provider. You can find & submit this in Week 5.
· Have them sign the form
· If they can’t sign it (like you are doing it virtually), type or write in their name, then put your initials. Example: Dan Casey/BH
