The Topic of my Community health problem is Teenage pregnancy in the Hampton Roads area (Virginia)
· To identify an existing community health problem.
· To identify the lines of defense in the community.
· To describe community health nursing opportunities at primary, secondary, and tertiary levels of prevention.
· To evaluate the adequacy of existing community health resources.
· To explain the role of the community health nurse in addressing the community health problem.
Method:
· Review a completed community health needs assessment and select an identified community health problem. Identify and describe the normal and flexible lines of defense that exist in the community. Identify existing community resources that address the selected community health problem.
· Describe the existing community resources in terms of the levels of prevention (primary, secondary, and tertiary).
· Evaluate the adequacy of existing community health resources. Are they sufficient for addressing the selected health problem? If not, what additional community resources are needed? (Think primary, secondary, and tertiary levels of prevention.) If so, what could be done to enhance usage of existing community resources?
· Explain the role of the community health nurse in addressing the selected community health problem.
Federal regulations require that tax-exempt hospital facilities conduct a Community Health Needs Assessment (CHNA) every 3 years and adopt an Implementation Strategy that addresses significant community health needs. Sentara and other healthcare facilities conduct a CHNA and publish the results for public review.
· Go to Sentara’s Community Health Needs Assessment website to select a completed CHNA for your own clinical site. Be sure to select the most recent CHNA. If you are not employed at Sentara, please contact your instructor, who will assist you in locating your own community’s CHNA, or you are welcome to select a Sentara CHNA report from the website.
· Identify existing local community health resources that address the CHP.
· Review the scholarly literature on your selected topic.
· Use a minimum of 5 scholarly sources published within the last 5 years.
· Interviews and general Internet sources are appropriate as additional references.
· You will then evaluate whether existing community resources are adequate to meet the health needs of the community.
· If existing resources and not adequate, you will discuss what improvements or additional resource(s) are needed to meet the health needs of the community.
· If existing resources are adequate, you will suggest how to improve or enhance the usage of the community program(s).
· The paper should be written using APA format, including a title page and reference pages, and should be a maximum of five (5) pages. The 5-page limitation does not include the title page and reference pages.
