1 Ineffective public transportation related to limited public (metro) transportation resources as evidenced by 4miles away from the health center location.

2 Risk of increased chronic health conditions in the Hispanics population of Cypress, R/T resistance in seeking help due to possible financial constraints, physical access to services, and lack of knowledge of the American health system, and language barrier, as evidenced by a decrease use of preventative health services. Health disparities exist in this vulnerable population, Hispanics reported delay in non-receipt of needed medical care because of cost concerns.

Instructions:

· Develop a potential plan based on either the top negative diagnoses above

· Identify goal(s) of the plan.

· Provided at least 3 evidenced based strategies for plan implementation

· Demonstrate how the plan can be evaluated
