Your Practicum experience includes working in a clinical setting that will help you gain the knowledge and skills needed as an advanced practice nurse. In your practicum experience, you will develop a practicum plan that sets forth objectives to frame and guide your practicum experience.
As part of your Practicum Experience Plan, you will not only plan for your learning in your practicum experience but also work through various patient visits with focused notes as well as one (1) journal entry.

Refer to the instructions in Week 2 to create individualized practicum learning objectives that meet the requirements for this course. These objectives should be aligned specifically to your Practicum experience. Your objectives should address your self-assessment of the skills found in the “PMHNP Clinical Skills Self-Assessment Form” you completed in Week 1.
As you develop your individualized practicum learning objective, be sure to write them using the SMART format. Use the resources found in Week 2 to guide your development. Once you review your resources, continue and complete the following. Note: Please make sure each of your objectives are connected to your self-assessment. Also, consider that you will need to demonstrate how you are advancing your knowledge in the clinical specialty.
Objective 1: My main objective is to achieve complete independent functionality when providing nursing
and healthcare services. One of the areas that I will be focusing in learning more on how to
effectively differentiate pathophysiological and psychopathological to improve part of the skills that
I desire to have (Curtis, 2021). Successful performance in activities that surrounds our daily lives so
as to have an independent life is critical particularly when it concerns the life of a patient who may
require professional assistance in executing their daily activities. it must be understood that
Having adequate skills and knowledge is imperative to living in a social world as they
facilitate us to perform our basic duties. Having self-evaluated by competence and most valuable
skills that I may need to successfully provide standard clinical services as required, I feel that I will
be more focused to establish the most effective and appropriate personalized patient care plan with
respect to the patient's response. As a nurse, my objective to determining an applicable customized
patient's treatment plan will be guided by the behavioral response of the patient to the provided
nursing interventions, and error identification in the clinical plan of care (Curtis, 2021).

Objective 2: I will be also focused to acquire more skills on how to professional develop an
age-appropriate customized care plan. I am planning to achieve this by creating adequate to learning
much about the routines based on the individual or family's cultural preferences and practices related
to sleeping a resting, eating or feeding, diapering and toileting so as to be able to establish
individualized care plan the schedules patients’ routines that meet their personal healthcare needs
(Curtis, 2021). Having adequate knowledge and skills in the development of personalized care of
plan will not only facilitate delivery of quality and effective healthcare services but will also ensure
patients experiences and expectations are met and perhaps ensure that there is adequate provision of
information and resources to the family or individual to help talk about their concerns whenever
need be. I believe this can be achieved if I devote my efforts towards assessing the patient’s health
needs, determination and listing of noosing diagnoses, setting goals that are ideal with the patients’
needs and evaluating of nursing care progress, and make the adjustment to the care of as may be
needed (Klemenc-Ketis & Poplas-Susič, 2017).

Objective 3: All Clinical settings ought to have screening tools to help safeguard patients and perhaps
facilitate the determination of areas of practices that may require adjustment. Since I don't have
confidence and adequate skills in the use of screening tools my objectives will aim at gathering
more knowledge and experience from both colleagues and continues learning to establish the best

ways to implement the screening tools and instruments more efficiently and effectively with my
clients to assess their healthcare needs (Iragorri & Spackman, 2018).

This timeline is intended as a planning tool; your actual schedule may differ from the projections you are making now.
I intend to complete the 144 or 160 Practicum hours (as applicable) according to the following timeline/schedule. I also understand that I must see at least 80 patients during my practicum experience. I understand that I may not complete my practicum hours sooner than 8 weeks. I understand I may not be in the practicum setting longer than 8 hours per day unless pre-approved by my faculty.
