Health Behavior Change - Client Journal

Overview

This health behavior journal will focus on the sugar intake of my friend, Sami. Sami is 27 years old who lives alone and very close to university. He studies engineering science at the University of Toledo. He is always busy and depends mainly on fast food and cold drinks. Sami comes from a family with a history of Diabetes. His father got diabetes at a very young age of 25 years old and his mother got it at the age of 32. Among his siblings, his younger sister is overweight and going through the diabetic monitoring through the hospital. Sami typically spends his morning skipping his breakfast and opting for quick energy bars or juices before he rushes for his classes. As he spends most of his days attending classes, he usually keeps his snacks to go which are mainly unhealthy. His lunch usually contains a smoothie or to-go fast food burgers with cold drinks. He also repeats his meal patterns for dinner and sleep right after his meals, which doesn’t allow him to properly digest his food. On an average, he drinks over a liter of soft drinks and easily about 300 grams of candies and chocolates. I have also recorded his intake of a pint of ice cream for dinner. He also managed to replace water with cold drinks. Other than his food activities, his physical activities are about minimal to non-existing. Observing his health behaviors and habits, it can be examined that his daily intakes of food and his physically inactive life at this rate is not going to be good news for the future. In this journal, we are going to focus on the major issue of the sugar intake which might lead him to be a diabetic. The smart goal of this project is to: reduce the sugar intake and improving the food habits using the health behavior theories. The objectives of this project that will be applied gradually within 6 weeks are:

· Objective 1: The client will be able to reduce the intake of sugar.

· Objective 2: The client will maintain mealtimes and have alternatives for sugar products.

· Objective 3: The client will know regularly how to read and monitor blood glucose and calories.

I have also conducted a S.W.O.T analysis:

Strength

Weakness

· He believes that his determination is strong.

· Excited to the outcome, especially since he is close to graduation.

· He hates the long-waiting time for food preparation.

· No knowledge of how to read food labels and calories.

Opportunities

Threats

· Roommate is willing to motivate him and participate for him to reduce the intake.

· Family is willing to financially support him to overcome his habits.

· Fast food restaurants are right next where he lives.

· Free soft drinks and snacks at engineering labs.

Despite Sami is busy with his school at the university, Sami and I agreed to meet twice or three times a week, also, communicate through the social media and texts, to provide as much information as he can about his progress.

Week 1: It will be easier if you start early!

Weekly Goals

During this first week of this project I plan to assess his behavior using the constructs Transtheoretical Model (TTM), found by Pochaslca and DiClements. From my dissection with Sami, I concluded that his in Preparation Stage (stage 3), according to Stages of Change, TTM. By using the model construct:

· I will try to replace his Snack time with a salad that I promised to drop it by and join him during his lab break at the university. by taking this action at least I could replace his king Snickers bar size, which contains about 44 grams of sugar with lower intake of sugar. Based on his weight, his sugar-gram daily intake should be between 30-37 grams (≈150 calories).

· I’ll also explain to him why monitoring the blood glucose is important.

· We also, going to look for alternatives to his candies, by writing down all his other favorites.

Measurement of Performance for 1st Week

In this week, Sami wasn’t able to have readings of his glucose level because he doesn’t own glucose level meter device. As for his sugar intake, he was able to start recording his intake from Wednesday. In this week performance we find, Sammy began to take it seriously and started recording his approximate sugar intake and looking at the nutritional components/ingredients of his food.

How I Used Material from the Course

I decide to use Theoretical Model (TTM) for this week. Pervious this week when I spoke with Sami I found him in contemplation stage (2nd stage) also known as “behavioral procrastination stage”. He was seriously considering changing his behavior, but he thinks it would be better to change after his graduation which is this semester. Also, Sami wants to change sooner but he believed his scattered thoughts about his habit need time to be organized, so he can know from where and how he should start. He believed he wasn’t ready yet. However, in the 1st week, he started to prepare for it and being more serious about taking actions. By sitting down with him during his Lab’s break and creating personal action’s plan, Sami’s moved to contemplation stage (3rd stage), In Process of Change (POC) term, his consciousness level is raised after our breaks’ conversations.

Description of Thoughts, Feelings, Emotions, and Barriers

In this week, I felt that my friend was enthusiastic to change because he had ideas to change, but he needed guidance. After stating that he would be very happy if he started from now rather than after graduation so that after he graduated he would be on his track of maintaining the change, and he only to worry about is looking for a job. Moreover, from the conversations we had I think the major barrier my client facing is time management. Once, he mentioned that he is generally aware of the risks of him getting diabetes, but he always thinks still have time to change in prevent it in the future, so it’s not important to work on these habits now. But by arranging his life schedule and finding alternatives I expect he will overcome them and get what he wants.

Goals for Next Week

· I’ll tech my client his normal blood glucose range and how he can measure it.

· I’ll help my client with providing ways to organize his life schedule, so he can have proper time for his meals.

· I’ll also substitute one of his snack/meal with a vegetable.

Week 2: Numbers matter!

Weekly Goals

During this week to achieve the weekly goals, I’m going to apply Theory of Planned Behavior constructs. Although, I will be only meeting Sami twice this week because he has midterm exam, but we will have at least 3 video calls with him.

· On our second meeting we’re planning to buy a smart blood glucose meter from CVS pharmacy. I’ll also started to explain to him that his normal rang for blood glucose and sugar intake (the maximum blood glucose while fasting or on empty stomach is 100 mg/dL, and the maximum blood glucose after eating is 140 mg/dL). For this project, Sami’s normal range will be between 100-140mg/dL. Sami will start measure his blood glucose daily on empty stomach for accuracy reading, and if he couldn’t, he will try to measure it after couple hours from eating. Although, this point does not affect as long as Sami’s reading is in the normal range or below 140 mg/dL.

· Next, while video calling Sami, I’ll suggest him to substitute one of his of daily Snickers bar with baby carrots.

· As for his third weekly goal, me and my client will be looking for a phone application that organizes and mealtimes and remined you of it.

Measurement of Performance for 2nd Week

As mentioned earlier, the sugar intake it may not be accurate for this project, but since Sami insisted that he learn about reading food labels and loved it, why not. loved the readers of the ingredients of sweetness. Although, it is possible to take a usage of this information in the future, but our major focus is his blood glucose level, and our minor focus is hi sugar intake. Sugar intake is being monitored to get an idea of how my client sugar intake increases/decreases.

How I Used Material from the Course

I decide to use Theory of Planned Behavior for this week. I specifically applied the Control BeliefsPerceived Power constructs of the theory. Sami loves modern technology. Therefore, when I was choosing a smart blood glucose meter device, I bought an advance unit where it connects via Wi-Fi to your iPhone application and save all your records on one application, so you can access them anytime. By doing this I applied Control Beliefs and Perceived Power construct because to Sami “If it will make it easier on me to monitor all these numbers and have them in one my phone without writing them on papers, I’ll buy it”- Sami. By doing this action I felicities the likelihood of increasing my client to record his number.

Description of Thoughts, Feelings, Emotions, and Barriers

I felt my client had the ability and the desire to change but his Perceived Power and Control Beliefs were missing. I noticed, more of Sami words are shown that he can change, but he doesn’t like writing everything down, losing them, then trying to remember the most recent records...etc. On Monday Sami, once told me that idea of him changing his behavior before graduation is began to get him excited. Also, my clients got really excited as he knew there’s a fast-way for synchronizing all the records to his iPhone instantly. Moreover, between the conversation lines, Sami showed me that I can trust him on substituting his one of his Snickers bars with baby carrots. I think my client showed me a good start this week. I’m excited to see his behavior changes for next week.

Goals for Next Week

· I’ll tech my client his normal sugar intake and how it correlates with his blood glucose level.

· I’ll help my client to use the iPhone calendar application as a meal reminder for his mealtimes.

· I’ll also try find soft drink alternatives.

Week 3: Now, timing matters!

Weekly Goals

In this week, I’ll be applying Health Believe Model. I think the most challenging thing my client will face this week is mealtimes. As substituting his soft drink, I don’t think Sami may face a hard time doing that, hopefully. I’m going to meet Sami 3 times in-person this week. Perhaps we work on one goal a day, to deliver the best of information and change.

· First, I’m going to explain to my client that he can use the iPhone calendar application as mealtime reminder, YES that’s right. It’s easy, all he has to do is set up the mealtimes he would like. I suggested to add only breakfast as for now. Although, Sami barely eat prober breakfast, but we are working on that. After that, he can choose between an alarm on exact time he set or a 30 minutes reminder notification. He can set up recurring reminders for each day for the rest of the year. (via an option on iPhone).

· Moreover, I’m going to teach him his normal range of his sugar intake by reading his snack labels. In depth, I’m not expecting my client to have an exact accurate (in grams) sugar intake but monitoring his sugar will give Sami an idea how much he eats and how many times he passes his daily normal intake. My client will be looking at his food labels and just try to add the number in his head, they don’t have to be accurate but close enough to be acceptable. As mention previously, Sami’s maximum daily sugar intake is 40 grams.

· Last but not least, I’ll take him out to go to JambaJuice, it’s a fresh squeezed juices store. I picked this cause it’s close to the university. I’m planning to get Sami, the Açaí Super antioxidant smoothie, which has candy like flavor.

How I Used Material from the Course

This week I used Health Belief Model, that was found by Godfrey Hochbaum, Irwin Rosenstock, and Stephen Kegels in 1950s. Initially I used Perceived Barriers construct. Sami’s negative outcome was time management. Sami’s fear is that he has a busy schedule where he may miss mealtimes he set up. But majorly, I’ve applied Cues to Action construct of the model. By setting daily breakfast and lunch time reminders, I’ve applied an external event that motivates my client to act. I hope these reminders work best for my client.

Description of Thoughts, Feelings, Emotions, and Barriers

I feel my client is feeling fear that not finding time for his meals, but after providing him a way of managing his mealtimes with getting reminders that shows in his iPhone will make him engage with it. Plus, in previous week we found a way of him getting all his blood glucose records in his phone. I asked Sami, what he really thinks about being in 3 weeks. “my feeling now is more to be focus on staying on the track and developing step by step… I got everything now in my hand (iPhone) I must take it seriously.” Sami said. At this week, my client shows that he feels responsible for his future.

Goals for Next Week

· I’ll add an actual complete healthy lunch meal to my client calendar.

· I’ll help my client preparing meals at home instead of buying fast food.

· My client will be able to substitute another of his snacks with a healthier option.

Week 4: The secret ingredient is patience!

Weekly Goals

In this week I’ll be using The Social Cognitive Theory (SCT) that was found by Albert Bandura. Using this theory my client’s behavior will evolve from Social Learning, self-efficacy constructs and more. From pervious conversions I had with Sami, I noticed that he has no problem to cook, since he knows the basic cooking, but he prefers not because it requires a lot of preparation time. Although, I don’t see my client facing an issue of adding lunch meal to his phone reminder since we did this by adding breakfast to his calendar last week.

· This week will be adding lunch mealtime to my client calendar. Previous week, adding breakfast to my client calendar wasn’t hard. My client said he missed only one breakfast, and the reason why it was easy for my client is to stick with that breakfast is easiness. For Sami, boiling eggs and potato, some steam carrots that didn’t take more 30min for him is convenient. In the other side, adding big meal as a lunch is huge change step to my client, but I’m confident he can make it.

· The biggest challenge to my client for this week is going to be preparing food. Sami, most concern is “I won’t’ have time to cook for 2 hours”. So, my plan is I’m going to introduce him to another iPhone application called myPlate. This application has 25 minutes or less meal, including cooking and preparation time. Following this, the application will ask you couple question while setting it up and can also send your grocery-list every Sunday based on you choose of meals. Either you choose the meals for the coming week, or you can keep it random.

· Last, I’ll suggest my client to substitute his snickers chocolate bar with peanut butter along with his baby carrots from 2nd week.

Measurement of Performance for 4th Week

In this week performance measurement, Sami was able to maintain his daily sugar intake for at least 3 days in row. Also, you can see how this impacts his blood glucose level graph.

How I Used Material from the Course

In this week I chose to use The Social Cognitive Theory (SCT) for its concerts. I applied Outcome expectations (Expectations & Expectancies), Observational Learning, and Facilitation constructs. My client expects that if he follows myPlate healthy food suggestions, his blood glucose level will be low, and he will be able to reduce the chance of him getting diabetes. Also, Sami value how this application is organizing and suggesting all low sugar meals for him (Expectancies). Once I suggested my client, once a week to visit his family and observe his mom cooking and learn from her. By doing this action I applied observation learning construct. Last facilitation construct, by providing this iPhone application as source for my client this construct is applied.

Description of Thoughts, Feelings, Emotions, and Barriers

I feel this week clearly shows that my client is initially refusing anything that takes most of his time, like food preparation which it was his main barrier. On the other hand, my client gets excited when there’s a shortcut or a tool that saves a lot of time for him. I personally believe Sami will be able to overcome his barrier, but he needs to be patient. I also, think this time he is excited but not so happy about it. Although, he told “I’m more serious to change so I have to accept it, so I can change”. After, listening to this I did understand what my client is going through which is he is in the process of convincing himself to accept that change.

Goals for Next Week

· I’ll add an actual complete healthy dinner meal to my client calendar.

· My client will be able to substitute another of his soft drinks with a healthier option.

· My client will go more than once a week to have a family healthy dinner.

Week 5: 3 main meals are completed!

Weekly Goals

During this week, week 5, I decided to use an interpersonal commination theory which is Transactional Model of Stress and Coping. I picked this theory for many reasons. In the previous weeks, I noticed my client he follows with all ease and he takes whatever suggestion I give him without any questions, regardless of me giving him reachable information and alternatives ways to overcome his bad behavior. This behavior worries me a bit, and that’s why I decided to use this type of theory to help Sami to become an active seeker for the information but with less distress. It’s a challenging week.

· In the end of this week, my client should have 3 main complete healthy meals, breakfast, lunch, and dinner which it will be added this week to my client’s calendar. As my client wish, this week Sami will be having light dinner which contains: 1 cup steamed vegetables with lentil soup.

· Second goal for this week is to replace his soft drinks completely. Last week my client substitute one of his drinks with Acai which he can buy at JumbaJuice store. Although, this time I’m teaching my client to depends on himself of seeking healthy juices recipes online and make them at home. So, I’m introducing my client to juicerecipes.com website.

· Also, Sami will visit his family twice this week for proper healthy dinner time.

Although, Sami missed couple of his blood glucose readings this week but I’m happy with his sugar intake results.

How I Used Material from the Course

This week, I am applying the Transactional Model of Stress and Coping. In my previous conversations with my client I noticed he just accept whatever I provide him as an information, not to say this is bad but I was afraid this may reflect on his independency. Sami once mentioned, the reasons of him not taking an action yet was the searching for the right information and will this information will be the ultimate one that fits him! It’s more if he has to go through a lot of searching that will distress him. So, by making Sami search through juicerecipes.com website for juices recipes and finding what actually suits him I’ve applied the Monitor construct where my client becomes an active seeker but also reduce the distress.

Description of Thoughts, Feelings, Emotions, and Barriers

My feelings about this week, I think my reachability to Sami makes a bit dependent. However, I can’t be certain about this but although it’s a possibility to me I decided to apply the Transactional Model. Moreover, I think Sami this time enjoys being with his family, but he didn’t realiz that he missed his healthy mom food dinner time till he visited for the second time. My client also told me that he felt heavy footed going to his family house for the first time but it’s bad at all for him for the second time. I’m so confident that my client will keep this habit going due the excitement he showed with for his coming family Thanksgiving dinner. As barriers, my main client’s barrier for this weak is seating down and reading through all the juices recipes and make what’s healthy with counting his sugar intake.

Goals for Next Week

· I’ll assess and teach my client how to maintain his changed behavior.

· I’ll add natural-sugar honey to my client snacks as treat.

· My client will have “cheat day” once a weak in the end of this project week.

Week 6: The most difficult task!

Weekly Goals

In this last week, I decided to use Social Support and Social network Theory. I believe this theory will have a big impact on my client maintaining his changed behavior, but he has to understand it first. In this last week I’ll be focusing more on teaching my client how to continue his journey on his own as well as how to maintain what he accomplished so far. On the other hand, I want my client to feel it’s normal to feel discomfort for while after changed but in order to reduce this feeling he has to use social support.

· Maintaining a changed behavior is the most difficult task but with Geographic Dispersion, it will be easier on Sami. For professional results, I’ll make my client sign up with nutritionist that he can reach for counseling, that’s near where he lives.

· Although, I believe my client had a strict journey so far. So, I believe adding one spoon natural-sugar honey spoon every morning as treat with his breakfast will make him less craving for candies or sugary snacks. Last but not least, the most important add is cheat day.

· I’ll also add a cheat day to my client once a week. In this day my client will be allowed to eat anything, either dinning out home or ordering from outside but with small rule which is paying attention. I’m not planning to ask me client to monitor his sugar intake during this day nor glucose level, I’ll leave it up to my client, but he has to keep it in the back of his mind that he should be aware of how much he is eating on this day.

In this week, we can see Sami has entered the phase of controlling his habit most of his records are in the range of his normals.

How I Used Material from the Course

I used Social Support and Social network Theory in this week. I’ve applied Geographic Dispersion by finding my client a near service to where he lives, so he can refer to anytime he needs more trusted sources. I also, have applied Social Undermining by explaining to him, that’s it’s going to get harder and people always will through criticism on you because they are either jealous or they couldn’t get to your level of maintaining it. Unexpectedly, I think I’m joining my I might pick up my client journey as well. Seeing the significant change in just 6 weeks, kept me thinking to adopt this program. Thus. I guess you can say I got influenced by Sami. Social influence has been applied.

Description of Thoughts, Feelings, Emotions, and Barriers

In this week, my client did not show me any barriers, although it doesn’t mean he didn’t have any, but I’m really sure it wasn’t severe although it would be shown. As my thoughts and feelings, I admit that I’m really amazed how just 6 weeks can show big changes. I feel my client will never regret this change. Moreover, after sitting down with Sami and having a nice conversation while eating vegetables with him, my client made me feel that he starts to feel more active and he can maintain it. Second thing he showed me is, his excitement to visit a professional nutritionist. Sami is planning to go visit a nutritionist and present all his recodes and intakes of sugar and discuss it and see what they think about his journey. I think Sami is starting to get influenced which I like and enjoy because I can see the approve that behavior change using theories really work.
