These teams can be most effective when they share responsibilities and foster role interdependence.

The issues include medical errors, long hours of waiting at the facility, high cost of care, and privacy.

This is a primary health issue that was identified at the National health facility.

It is a leading cause of mortality and patient harm within the facility.

Particularly errors occurring during surgery and transplantation.

Guaranteeing patient safety is fundamental, so it is essential to solving issues revolving around medical errors.

Transplantation errors- ABO and graft incompatibility.

Surgical errors- surgical injuries, wrong surgery site, mistaken patient identities.

Interdisciplinary team collaboration plays a vital role in facilitating communication among healthcare providers.

The interdisciplinary team is a vital part of patient care planning.

Using an interdisciplinary team will enable healthcare providers to work in harmony, communicate issues before delivering care.

This will help reduce surgical and transplantation errors, patient harm, thus improving patient health outcomes.

To enable the healthcare providers to improve the communication between them.

To correctly identify patients by verifying their identity.

To identify patients’ risks and safety dangers before surgery or transplant.

These objectives will work when there is effective communication among healthcare providers.

With effective communication, the objectives will be achieved, which will reduce mortality rates and patient harm.

The identity must be confirmed in at least two ways, i.e., confirming the assigned medical record number and the photo.

The Role of the Interdisciplinary Team in implementing the objectives

The head of surgical and transplantation incorporate training on effective communication.

In addition, he will make communication part of the facility’s culture.

Nurse X will ensure all healthcare professionals are taught the effects of medical errors and how to reduce them.

Nurse Y will teach others how to identify patients, correct donors, and grafts correctly.

The head surgeon will advise the laboratory technicians on counterchecking the results before dispatching.

Implementation and Resource management

The plan can be implemented by educating all healthcare professionals on patient safety and reducing medical errors.

More highly-skilled staff can be employed to help curb staff shortage.

All the available surgical, laboratory, and other medical equipment are maintained more often.

Purchasing of new equipment to replace the obsolete ones

The plan can be managed by identifying and noting the areas that need improvement and budgeting for them.

The plan provides the amounts required for each resource.

Implementing and managing the resources is essential in ensuring the objectives for change are met.

Evaluation

Reduction in surgical patients’ death will reveal the success of the plan.

Successful surgeries and transplantation for most of the patients.

Reduction of adverse health events to patients receiving an organ transplant, surgery, or any other medication.

Improved and effective communication between healthcare professionals.

Reduced cases of conducting surgery and transplantation on wrong patients and wrong sites.

Realizing all these will show that the plan was successful, and no resources were mismanaged.

All the healthcare workers should accept the inherent issues in their roles as healthcare professionals that influence error-prone work environment.
