Social Determinants of Health

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

5

Policy Implications of Social Determinants of Health

Policies affecting social and economic determinants of health are ultimate prevention policies

“Fundamental causes” of health

Help to shape the contexts in which individuals live their lives and have significant health consequences

Education

Poverty

Housing

Employment

8

Policy Implications of Social Determinants of Health

What are upstream social determinants of health?

Nonmedical, social, and economic factors, as well as environmental conditions

They are gaining interest, but policymakers and individual consumers continue to invest most of their attention and resources to pay and deliver health care

More $$ healthcare but lag behind on health indicators

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

9

Effective policies that consider not only the delivery of health care services, but also social services, education, employment, and the built environment are the most promising for preventing illness, injury, and premature death

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

Largest impact on population health will be a result of providing equal access to fundamental resources

10

Social and Economic Policy and Preventive Health

Social and economic policy interventions will likely improve the well-being of children, youth, and families

Early Childhood

In comparison to households children who live in poverty

Complete fewer years of schooling

Work fewer hours

Receive food stamps at higher rates

More likely to report poor health

About 50% more likely to be overweight in adulthood

Early childhood care and education policies to increase accessibility for low-income families:

Head Start

Child Care Development Fund

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

11

Social and Economic Policy and Preventive Health

Education and Policy

Those that help prevent school dropout and support higher educational attainment can also function as preventive health interventions.

Increase in years of education results in living longer and healthier lives

1% increase in of Americans with some college education could save $1.3 billion per year in avoidable medical care for diabetes

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

12

Social and Economic Policy and Preventive Health

Education and Policy

Students’ health also influences their ability to learn and to complete their education.

School-based health clinics/centers (SBHCs) have been shown to Improve access to services students might otherwise find challenging

Users of SBHCs had higher completion rates for immunization series, reported greater satisfaction with their health, and engaged in a greater number of health-promoting behaviors.

Access to SBHCs was associated with a decrease in the rate of hospitalization and a gain of three days of school for children with asthma

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

13

Social and Economic Policy and Preventive Health

Asset Building

Child Development Accounts (CDAs) are savings or investment accounts opened as early as birth. Goal is to that children have the resources to reach milestones such as postsecondary education, homeownership, or starting a business

Longitudinal study

CDA leads to better social and emotional development for the most socially disadvantaged young children who received a $1,000 CDA at birth

23 Mothers of children with CDAs in the Oklahoma experiment also report significantly lower levels of depressive symptoms

Currently 3 states with policy:

Maine, Rhode Island and Nevada

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

14

Neighborhood, Community, and Economic Development

Living in a neighborhood with a low socioeconomic status (SES) is associated with increased risk of mortality compared with living in a neighborhood with a high SES

Community development policies that promote better housing options have been shown to enhance safety and reduce the likelihood of being exposed to crime

Under the Affordable Care Act (ACA)

Established National Prevention Council which issued the National Prevention Strategy

Focus on housing quality and cross-sector collaboration in community development planning to improve health

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

15

Neighborhood, Community, and Economic Development National Example: Education & Health Initiative (EHI)

Led by Virginia Commonwealth University Center on Society and Health

Raise awareness about the implications of educational attainment for health outcomes

Message for leaders in health policy was better education could reduce disease rates and help control the rising costs of health care

Message for education leaders was the health benefits of improved education could strengthen reasons for policy and funding

Most importantly:

Eyler, Chriqui, Moreland-Russell & Brownson, 2016

16

Bridge the disconnect between researchers and policymakers

Neighborhood, Community, and Economic Development National Example: Education & Health Initiative (EHI)

Research

Scientific findings of the relationship between education and health with four sets of materials

Multiple products (issue briefs, videos, web content) aimed at specific audiences

Establish relationships to help with dissemination of materials and introduce researchers to the Center for Society and Health as a resource for information on connections between social factors and health

Eyler, Chriqui, Moreland-Russell & Brownson, 2016
