Change Strategy for Vila Health

At St. Anthony Medical Center, all operations integrate advanced measures to address patients' conditions and conduct accurate reviews to assess their treatment adherence capabilities. Using information from a concept map developed for Villa Health, a specific patient, Mrs. Smith, was admitted with high glucose levels ranging from 200 to 350+ and Acute Renal Failure. Various interventions were provided to her during her hospital stay following an assessment of her condition. Upon returning home, the patient's blood sugar levels ranged from 125 to 140, indicating a reduction due to adherence to the treatment methods provided during her hospitalization. Her most urgent diagnosis was acute renal failure, characterized by symptoms such as shortness of breath, blurred vision, and leg swelling. Improving health assessment accuracy can be achieved by focusing on general health practices necessary to address patient risks and then determining appropriate interventions (Bachman et al., 2020). Based on the patient's condition, implementing improvements according to the initial findings and establishing expected interventions is valid.

Immediate interventions focused on medication use and strict electrolyte management. Addressing a patient's condition is essential in creating a reliable treatment plan that adheres to public health standards of care (Dembek et al., 2018). Another diagnosis the patient had was hyperglycemia, which led to symptoms such as blurred vision, fatigue, and an HA1C reading of 8.5. Proper interventions for the patient included administering insulin therapy and encouraging daily physical activity. Although there was a change after implementing these interventions, further change strategies are necessary to facilitate more advanced treatment methods for the facility.

Immediate change is necessary at the facility to implement better treatment methods. The initial strategy involves establishing a multidisciplinary team consisting of pharmacists, dietitians, nurses, and endocrinologists. Working with experienced healthcare providers is highly imperative as a change strategy, as it provides access to diverse ideas aimed at improving patients' health conditions (Cakiroglu et al., 2022). Collaboration among all the mentioned providers will involve monitoring patients' blood glucose levels, providing education on lifestyle modifications, and adjusting medication to promote better health outcomes. After Mrs. Smith, the patient, visited the facility and was subsequently sent home, she was expected to follow additional treatment protocols. This multidisciplinary approach is appropriate because it allows for addressing a patient's complex needs from the unique perspectives of different providers.

Another effective strategy is the use of Continuous Glucose Monitoring (CGM) technology, where patients wear a CGM device at home (Miller, 2020). This technology provides essential information about glycemic control, which is crucial for determining appropriate interventions. The approach is beneficial as it offers accurate data about blood glucose trends, aiding in early detection and management. Patient safety operations can be significantly improved by assessing recorded trends and promptly implementing necessary interventions.

Implementing a renal support protocol is another effective strategy, wherein patients receive close monitoring of their renal function and optimization of fluid and electrolyte balance. This targeted intervention ensures timely recognition of physiological changes and consistent handling of patient needs through timely interventions. Timely data can be utilized to implement necessary changes whenever patient health improvement is required.

A further tactic is to create a symptom management pathway for patients with renal failure and diabetes to record all their symptoms. This strategy can then be implemented to tailor a treatment plan that ensures a structured approach to addressing patient needs. Using a personalized approach for each patient is possible. Common symptoms include nausea, fatigue, blurred vision, swelling, and overall weakness. If any of these symptoms are identified, a treatment plan will be developed and implemented, with regular reviews to assess the patient's condition for potential changes. This strategy is appropriate because it establishes standardized assessment methods, which are crucial for improving patient comfort levels (Rafferty, 2018). A proper health improvement process can be achieved when symptoms are assessed in a standardized manner, determining whether a patient's condition has improved after identifying abnormalities. All identified symptoms occur when a patient's status is abnormal, indicating that any interventions implemented will ensure better patient outcomes.

An additional approach is patient education, which provides a solid foundation for completing treatment for patients and overall improvement in their health status. When patients receive knowledge about their care needs, it leads to physiological changes where their bodies become more receptive to healthcare advancements (Rafferty, 2018). This strategy is appropriate because it enables patients to engage in self-management practices necessary to advance care using existing treatment plans. Education can be delivered through workshops or one-on-one sessions, empowering patients to become active participants in their healthcare. It is important to educate patients about the symptoms that indicate the need for immediate healthcare interventions, given the urgency of diabetes and renal failure. Using simple language during patient education sessions ensures patients understand and can implement the necessary treatment methods. Emphasizing the critical importance of maintaining adequate blood glucose levels during patient education sessions ensures that all treatment needs are addressed effectively.

A patient affected by Hyperglycemia can further be affected by renal failure. Immediate action is expected to improve the health status, and this has currently been done for the patient's situation using the following outcomes:

· Blood glucose management

· Renal function

· Improvement in patient monitoring operations

· Handling of patient-engagement operations

· Reduced risks of complications

Patient improvement is essential to ensure health status improvement, focusing on the following strategies:

Blood Glucose Management:

· Implementing an individualized care plan encompassing medication regimen, physical activity guidance, and dietary recommendations.

· Utilizing continuous glucose monitoring technology, enabling patients to monitor their glucose levels with a device placed on their arm, with readings transmitted to healthcare providers for further assessment.

· Providing comprehensive patient education to facilitate proper lifestyle modifications and medication adherence.

Managing Renal Function:

· Implementing regular screening protocols for early detection of renal issues.

· Establishing standards for electrolyte management to detect and address imbalances promptly.

· Educating patients on lifestyle modifications such as regular exercise and reducing sodium intake.

Patients adhering to the strategies are anticipated to experience improved conditions, including:

· Reduction in fasting blood glucose levels to less than 150.

· Enhanced patient adherence to medication.

· Implementation of lifestyle modifications contributing to overall health improvement.

· Improvement in renal function.

· Absence of symptoms like nausea, dyspnea, or fatigue.

For all the change strategies, difficulties may arise in terms of the strain on healthcare systems to access the resources required for adapting to new directions. Furthermore, the interventions can be costly and may not be applicable to all individuals who may lack the funds to purchase technological devices. Since these difficulties can contribute to health disparities, addressing them is crucial by thoroughly reviewing social and economic inequities such as limited education, safety in social contexts, and the lack of healthcare system development, so that new ideas for change can be implemented (Ndugga & Artiga, 2023). Forming collaborative health development sessions at hospitals and sharing discovered information with patients will improve their overall health conditions.

Using the data table above, it will be possible to improve patients' health conditions by addressing how the expected outcomes can attain improvements by gradually addressing the existing conditions. Healthcare providers can participate in meetings where they share ideas and explain their rationale for care. These meetings will present ideas for behavioral health equity that properly address differences such as sexual orientation, socioeconomic status, or geographical location of patients to provide them with health improvement (SAMHSA.Gov., 2023). Complete health improvement will be possible by focusing on access to educational resources that providers can use and those made for patients' academic development (Oster, 2018). All these interconnected practices will improve the health status of various persons within and outside the facility once they are discharged.
