SEXUAL HISTORY INTERVIEW
Use the following questions to develop a questionnaire for conducting a sexual history interview. The questionnaire should consist of 20 to 30 questions.
Current Demographics
· What are the particulars of the client's life currently?
· Are there any current health concerns? Are there any current medications?
· What are the client's current interests, exercise patterns, recreations, and health maintenance?
· Drugs: nicotine, alcohol, recreational drugs
· Historical and current use patterns
· Use of any drugs for sexual enhancement or to heighten erotic arousal
· Effects the drugs in question have on person sexually
History
· What is the client's parental marital history?
· If divorced, age of client when parents divorced
· Is there any sense of a sexual relationship between the client’s parents?
· What are the parental relationships with the client as a child, adolescent, and adult?
· What is the client's parental education history?
· What about the client's siblings? What is the nature of those relationships?
· What is the client's religious background? What is the religion’s specific attitude towards sexuality?
· What was right and wrong? Are there any feelings of guilt the client carried that he or she can identify as originating from their religious training? What are positive influences the client can identify as originating from their religious training?
Sexual Education
· At what age did the client learn and who told client that baby grows inside the mother?
· At what age did the client gain a first understanding of intercourse? At what age did the client learn that the penis could go inside the vagina?
· At what age did the client gain a first understanding of menstruation?
· At what age did the client first learn about masturbation? STDs? Prostitution? Homosexuality? Condoms?
· In general, what kind of sex education did the client have? What was the moral, psychological, and emotional tone?
· How did the client’s father, mother, siblings, peers, and teachers contribute to client's sexual education?
· Are or were there questions about sex and sexuality that the client has always wondered about?
· In general, you are looking for is an account of how the client first learned about sex and from whom.
· The client’s feelings, impressions, responses, etc.
· What was the client’s impression of their parents' feelings regarding sex?
· How did religious beliefs apparently affect how sexuality was handled in the family, then and now?
· How was nudity handled in the family?
· Does the client remember first seeing their mother or father nude?
· What about nudity amongst siblings?
· What about impressions and feelings?
Child Sexual Play
· What was the nature of the client’s childhood sex play with girls and boys?
· What was the nature of the client’s play exhibiting? Examples include touching, playing doctor, and mutual showing of private parts. Was there more focused genital play?
· What was the age at which these experiences occurred?
· Was play ever discovered by adults? What was the adult's reaction? What was the client's reaction?
· What were the client’s feelings regarding sex play?
Masturbation
· As a child or adolescent, did the client ever engage in masturbation? If so, at what age? What were the patterns?
· Was masturbation the client’s first experience of orgasm and ejaculation?
· What were the client’s reactions to the first masturbatory orgasm?
· What did the client know about masturbation at this age?
· Were there concerns that masturbation would harm the client? Examples include growing hair on his or her palms, stunted growth, and damage to self sexually.
· What were the client’s feelings about masturbation? Did they involve guilt and shame? Did he or she feel like the only person doing it?
· Was masturbation solitary? Were siblings, cousins, or others involved?
· What was or is the cultural and religious perspective regarding masturbation?
· How does the client describe parental attitudes about masturbation?
· What, if any, are the current masturbation patterns? Are there any concerns or questions about current patterns?
· Does the client fantasize sexually during masturbation?
· Generally, how long does it take client to reach orgasm during masturbation?
Dreams
· Has the clientever had an orgasm while dreaming? For males, have they ever had a wet dream?
· When did this first occur?
· How often currently does client have orgasmic dreams?
Sexual Victimization
Note: Use discretion in asking these questions.
· As a child, did anyone ever touch the client sexually when they did not want them to?
· Has the client ever been forced into sexual acts as a child or adult?
· Who, if anyone, has the client ever had a sexual experience with who was also a relative?
· Has the client ever been forced to have any kind of sex with anyone? If so:
· What happened? With whom?
· Where? When?
· Whom did he or she tell? If no one, why not?
· What did he or she do about it?
· How did or has this affected him or her? Pay attention to any indications of guilt the client may have about these experiences, even if they are denied.
Menstruation
· Ask female clients at what age she first menstruated.
· What did she know about menstruation when it began for her?
· How did she handle it? How did the family handle it?
· Were there celebrations of womanhood, or was it considered a burden?
· What were and are the client’s impressions and attitudes about menstruation?
· Ask male clients how and what they first learned about menstruation.
Puberty and Adolescence
· What was the client’s dating and sexual experience during high school and college?
· At what age did the client first hug, kiss, experience fondling or petting, and intercourse?
· How old was the client when he or she first had intercourse? Who was his or her partner? What was the enjoyment level? Did he or she achieve orgasm? What were the circumstances?
· What are the client’s number and circumstances of partners during course of adolescence and college, including duration of relationships, frequency, sexual activities, and sense of quality of the sexual relationships?
· What about homosexuality? It is usually an issue of when versus whether. Think about how to ask the question.
· What, if anything, curtailed or enhanced client's sexual behavior during adolescence?
· What level of influence do peer pressure, social, and cultural tenets, religion, fear of STDs, and fear of pregnancy have on the client’s sexuality?
· What were or are the client’s experiences with pregnancies of self or partners? What were the outcomes?
· At what age did the client first leave home to be on his or her own? What were the circumstances or reasons?
Marriage and Current Relationship
· What are the client’s current sexual activities?
· If married or divorced, at what age did the client first marry? How long had client known spouse before marrying? Did they live together before marriage?
· How would the client characterize the relationship?
· During the first year or years of the relationship, how frequent was sexual intercourse?
· How does that compare to current levels of sexual activity?
· What is the frequency of sexual contact?
· What is the amount of time spent being sexually intimate with partner?
· What are the initiation patterns for sexual activity?
· What behaviors does the couple engage in? Examples include kissing, foreplay, oral sex, anal sex, and sexual positions. What are the client's favorite activities?
· What is the client’s and their partner's comfort level with the couple's sexuality in general?
· What about specific behaviors?
· What is the amount and clarity of communication about sex? What is the amount and clarity of communication during sex?
· What does the client find sexually arousing?
· How regularly and consistently are both partners orgasmic?
· When are the client and partner most likely to be interested sexually? When are they least interested?
· Is the client aware of a particular relationship between his or her emotions and his or her sex drive and sexual availability?
· What is the client’s level of satisfaction with the sexual relationship?
· What have been the best and worst periods sexually in the relationship? Why does client think they were so?
· Does couple use contraception? What form? Does the client have any concerns related to contraception?
Dysfunctions and Dissatisfactions
· If the client could change the sexual relationship, how would he or she do so?
· Are there any problems associated with sexuality in the relationship?
· Children: Are there any? How do the client and partner see the demands of parenting as affecting couple's sex life? If there are no children, does the client and partner want any? Does the client want children with his or her current partner?
· Has there ever been any extra-relationship sexuality? If so, what is the pattern?
· Does the partner know about the extra-relationship sexual encounters? Does the client know, but the partner does not know the client knows?
Self Image
Here we are looking for general adequacy and inadequacy issues.
· What is the client's general sense of himself or herself physically?
· What does the client like best about his or her body?
· If the client could change one thing about his or her body, what would it be?
· How does the client think his or her partner feels about the client’s body? Is it a cause for concern?
· Are there any uncertainties and concerns about the client's body; specifically, his or her genitalia?
· Are there any concerns about the client’s partner's genitalia?
· Sense of client's sexuality: Does the client consider himself or herself to be confident in his or her sexual abilities?
· Is the client’s partner confident in the client’s sexual abilities? Is the client’s partner confident in his or her own sexual abilities?
· What is the best sexual relationship the client was ever in? What made it the best?
Fantasies
· Does the client sexually fantasize? If so, what are the circumstances?
· How varied are the sexual fantasies?
· What is the nature of the fantasy? Examples include scenes, mini-dramas, slides, and movies.
· From where does the client draw fantasy material? Examples are erotic films, literature, and past experiences.
· Do fantasies represent unrealized wishes or unfulfilled desires?
· Does the client have fantasies that are disturbing to him or her?
Erotica
· What does erotic literature and pornography mean to the client? Are there any negative reactions?
· What is the client's degree of comfort and discomfort with erotic materials? Are there any prohibitions?
· What is the client's reaction to videos, magazines, books, adult bookstores, and live nude and sex shows?
· Does the client prefer any forms to others? What are the client’s partner's preferences?
HIV Education and Awareness
· If the client is sexually active and non-monogamous, regardless of sexual orientation or gender, the therapist needs to inquire about HIV status.
· Does the client have an understanding of HIV issues?
· Does the client take precautions and have patterns of safer sex behaviors? Does the client consistently apply these behaviors?
· Has the client ever been tested? What was that like? Why was the client tested?
· Does the client have any current concerns about his or her HIV status?
Prostitution
· Has the client ever paid another or been paid for sex? What were the circumstances?
Sexual offenses
Note: Use discretion in asking these questions.
· Has the client ever forced anyone to have any kind of sex with him or her?
· What happened? How did it make the client feel? Did he or she ever tell anyone about it? How has this affected him or her?
Concluding
· Ask the client if there are any questions that it seems like were missed in terms of their sexuality or sexual concerns?
· Is there anything else the client thinks you should know?
· Is there any aspect of the client's sexual history or current life that the client is curious about or that causes him or her distress or concern?
