STUDENT NAME: _______________________ CBC AUTOMATION & REFLEX PROCEDURES Worksheet Refer to your clinical site policies and procedures as necessary to answer these questions. 1. List the specimen requirements for CBC testing.

Anticoagulant/tube type

Fill volume (minimum)

Sample type (be specific)

Stability time frame (for testing)

Specimen storage (temp/how long)

2. List any rejection criteria for CBC testing (Please note ALL common reasons why a specimen

would be unacceptable for testing). 3. List the CBC parameters that have Delta Check protocols in use.

4. What parameter is most useful for determining if a specimen is mislabeled or contaminated (by investigating a delta check)?

5. Fill out the following table about RBC indices. PLEASE REFER TO YOUR SITE’S ANALYZER and how it determines the following parameters. You may refer to the following article as needed: http://www.uptodate.com/contents/automated-hematology-instrumentation

*NOTE: For all analyzers, EITHER the hematocrit OR the MCV is measured (and the other parameter is calculated from that measurement). If you answer the same for both, you will be marked wrong.

6. For each of the following parameters, list causes of true clinical variation leading to delta checks. This DOES NOT include conditions that lead to a LOW or HIGH value over a long period of time (weeks – months), like iron deficiency anemia or megaloblastic anemia (for example). Listed here should be conditions that lead to a RAPID INCREASE or RAPID DECREASE in values in a short period of time (hours – days). EXAMPLE: A common reason for a legitimate decrease in H&H is bleeding. Acute blood loss can cause a distinct drop in both the hemoglobin and hematocrit values in hours; this would cause a delta check when comparing current to previous results.

Example: acute blood loss

one answer for both

(HINT: Anything on the list below for question 7 shouldn’t be used above – those are all reasons for SPURIOUS specimen changes.)

7. Given each of the following specimen issues, list the CBC parameters most likely to be spuriously effected, and WHY. A. Clotted specimen: B. QNS specimen: C. Mislabeled specimen:
