Based on your chosen and approved case study from week one, document the following information. Utilize the following headings and address the information as noted.

• Client history including: biological, demographic, work, academic, family, social, development, spiritual considerations

• Screening: identify and describe two published addiction screening tools that would be useful for this client and his/her presenting problem

• Presenting problem and problem list; What is the referral source? What does the client identify as the concerns for counseling? What other potential concerns do you want to monitor that were not identified explicitly by the client? Develop a problem list of 5-10 items prioritized, in your opinion, by most to least concerning

The paper should be written in APA formatting in the third person and should be 3-5 pages of content. You are required to include at least three scholarly references.
