We will use this case study to help you think through the topics you will address in your population health paper. Answer the following questions about “Integrating Behavioral Health into Primary Care.”

You do not need to style this as a paper but please use full sentences. And, each question should have more than one sentence in the answer. Grammar counts.

1. What is the organization and what is the population ? Describe the population in detail with what you know about their demographics and/or their clinical presentation. Note: You might find this information in more than one place in the paper.

2. Define the goals and objectives to move the population to a healthier existence.

3. Describe the intervention including how people were brought into the pilot, who provided the pilot treatment/intervention, where did this take place? Were any behavioral models at work (e.g., Transtheoretical, Behavioral Economics, the Health Belief Model, etc.). If not, state that you didn’t connect anything to the models we discussed.

4. Identify measurable indicators including the outcome and any process measures.

5. Identify challenges the researchers had to address and if the challenge had a solution.
