Case Study
Ms. Tyra Brown is a 23-year-old African American female who presents herself in the emergency department (ED) due to “feeling depressed” for the past six months. Ms. Brown reports that she no longer feels like doing any of the things she used to enjoy, stating, “It all seems so pointless.” She can’t even bother to eat most days and has lost weight recently. Although she has no energy, she finds it difficult to both fall asleep and stay asleep. She reports getting only 4-5 hours of sleep every night. Even when client does manage to fall asleep, she never feels rested when she wakes up in the morning. She reports having difficulty at work as a computer support person because it is so hard for her to concentrate. In the ED, Ms. Brown reports that she finds herself crying almost every day and that she has thought about suicide. Client further states, “I don’t know why I have been feeling this way.”
Ms. Brown is admitted to the behavioral unit with the diagnosis of major depressive disorder (MDD). This is her first psychiatric hospitalization. Vital signs on admission are T: 98.2°F orally, P: 78, R: 20, and BP: 106/68. Client is 5’6” tall with a weight of 110 lbs. Ms. Brown is started by her psychiatrist on paroxetine (Paxil) PO once daily.
Based on the case study, do the following.
