Based on your chosen and approved case study from week one, and the screening and assessment assignment from week three, document the following information. Utilize the following headings and address the information as noted.

• Functional Analysis using the functional analysis video from last week’s discussion dissect the client’s substance use issues—what do they use, when do they use, for what reasons do they use, what triggers exist?

• Case Conceptualization Summary reflecting on the Functional Analysis above and Problem List from week three, how do you see the client’s issues coming together, impacting one another, and ultimately being maintained in his/her life?

• Diagnostic Impressions identify the diagnosis or diagnoses appropriate for this client, include appropriate codes and specifiers from the DSM-5; are their co-morbid diagnoses present? describe your differential diagnosis process in which you identified, but ultimately ruled out, similar diagnoses

The paper should be written in APA formatting in the third person and should be 3-5 pages of content. You are required to include at least three scholarly references.
