Diagnosis: Patient is diagnosed depressive mood disorder.

E.J is a 35 -year-old AA female came to the clinic complaining of been overwhelmed. Patient is well-groomed and able to provide all questions and information’s as needed. She maintained good eye contact throughout the interview. Patient married and divorced. She has 3 children (8,10,12) who live with her. She gained full custody of her children, after her ex-husband said he does not want to be in his children’s lives. She has a very good family support and very active in their community, volunteering in the church. Patient has a family history of mental illness. Reported that maternal grandfather manic depressive disorder. She reported that it is very stressful to raise three children on her own. It is also very difficult to be working full time and go to all the kids’ activities. She barely has time to take care of herself. She had recent episodes of been very moody and depressed. There is time when she spent most of the time crying in her room. When asked how her mood is answered “I am feeling sad” Patient denies being depressed but voiced being very anxious.
DIAGNOSIS: Patient is diagnosed depressive mood disorder.
PLAN: individual cognitive behavioral therapy is prescribed for three months. Follow up every2 weeks but if symptoms persist or worsened prior to 2 weeks, return to the clinic.
