12-year old white male presents to clinic with stomach pains for >6 months (2 bowel movements per week)

Height: 64.5 in, Weight 130 lbs, SMR level: 2

Weight hx: One year ago, Adam was 62 in. and 100 lbs. Has a 30-lb weight gain over the last year – mom is concerned.

Medical hx: Blood pressure and heart rate normal for age and height.

Family social hx: Parents have been divorced for ~1 year and he is unable to see his father often. Mom and Adam moved to new neighborhood and mom works late and unable to prepare home cooked meals regularly. Adam not allowed to cook when mom is not home

Family medical hx: Mother recently diagnosed with type 2 diabetes, father is overweight and has hypertension
