You need to have an introduction in one paragraph. Here, you will state who you are, and the company you work for, that is the CDC in the Vessel Sanitation Program. State what you have been tasked to do, which is to investigate on the case of foodborne illness in a cruise ship from Caribbean voyage to Miami. In the same paragraph, you can give a brief description of what the case file is all about.

In the second paragraph, start by giving the situation of the sick passengers in the ship. State the steps that the chief medical officer on board has taken since the situation arose. This will include the BRAT diet that he has instructed those sick and the rest of the crew to take.

Outline the sequence of events in the third paragraph by starting with the first illness case. State when it started and who it affected first, that is the 26 year-old newlywed white female from Philadelphia. You can also give a brief summary of what the couple had taken since the day prior to their departure from Philadelphia. Ensure you include all the meals they had been taking in that duration of time. This will help in finding out the possible cause of the foodborne illness.

At this point, include the table from the female’s food diary (table 1). You can also provide a key so that the table can be understood better.

In paragraph four, continue to give the situation of the passengers onboard. This is by giving the number of passengers who have had similar illness cases. This is from day two to day five. Include the situation on the fourth day of the voyage. Ensure to indicate the symptoms which are diarrhea and vomiting, loss of appetite and a low grade fever.

At this point, include the table that indicates the number of sick passengers form day two to day five. (Table 2).

In the fifth paragraph, state that even three crew members have been affected hence making them to be removed from duty. All the food handlers have also been removed from duty. Include that the case load has already reached the 2% threshold hence making it necessary to file the mandatory report of the situation to the VSP. Include the third table (Table 3) at this point. This table will indicate the food diaries at various ports.

Conclude by giving the proceedings of the voyage, giving details about all the ports of call. Indicate that all the ports remaining of call have been canceled and that the ship is en route to Miami to extend the number of the medical staff.
