Course Outcome:

Step into the role of the Case Manager

Using the case studies attached, plan the post hospitalization care for the client.  This should be holistic in nature, taking every element presented into consideration.  Students will treat this as is this client in at the local hospital in your geographic area.  Any and all healthcare resources that are used should be from real local healthcare organizations or facilities.  

Tips: 

1) Facility placements (if needed) should be appropriate level of care and maintain the clients autonomy and lifestyle choices (if not explain)

2) Include how all care will be funded using the info given  (Use the Medicare and Medicaid information in the State of Virginia), consider looking it up.

3) Holistic

Personal History

Mr. Mark Willis is a 70-year-old-man who became an international star, environmental activist, pilot, and now is a part-time college professor.

Social History

He was born in Chicago, IL. He attended college in Wisconsin, where he was a member of the Sigma Nu fraternity. He took a drama class in college and was hooked on acting. He moved to Los Angeles, CA in 1964 to apply for acting job. He eventually signed a contract with Columbia Pictures. He tired of not getting the roles he wanted and became a self-taught carpenter to support his first wife and their two sons. His luck changed when he was hired to build cabinets for the famous director George Lucas, who cast him in several pivotal roles, making him an international award-winning actor. He is also a private pilot. He began flying in 1960. His older sons are grown and married. He has been married again and divorced in 2000. He had two sons with his second wife. He married an actress in 2010. They are parent of an adopted 10-year-old -son. He has three grandchildren.

He is known to be one of the most private actors and guards his personal life. Deciding that he wanted to have an even lower profile he built a large home in secluded area overlooking a nearby lake.

Financial Information

Mr. Ford owns homes in Hollywood, CA, Jackson Hole, WY, and his wife owns an apartment in New York in the Trump Towers overlooking Central Park. He owns an additional 800 acres of nature reserve in Jackson, WY, that he plans to donate as a wildlife reserve. He owns numerous planes and helicopters.

He has Medicare Part A, B, D, and a supplemental policy from AARP to help with medical expenses.

He has the financial means to pay for any type of LTC that he requires.

Medical Diagnosis/Conditions

He has a history of a chin injury from a car accident in his 20's. He suffered a herniated disk in his back, which required surgical repair at age 40. He has chronic pain in his back and left leg following the injury/surgery. Two weeks ago, he fell while hiking and fractured his hip, broke three ribs, and had a punctured lung. After 7 days of hospitalization, he went to a skilled nursing facility for skilled nursing and therapy.

He takes Mobic and Cymbalta for chronic pain. He is currently has central line catheter for antibiotics because of an infection following surgery.

Additional Information

Mr. Willis desires to remain in his new home. He realizes he will need more assistance at first but is planning to ensure he has adequate care for future needs. He is concerned that he will not pass the pilot examination or fly again. He wants to be where he can help provide some of the care for an active young son. He wants his wife to be able to continue with her career without worry about his health care. He would like to be able to return to teaching part time at the community college. He prefers nurses to provide his care. They have a live in housekeeper and part-time nanny. His desire is to maximize on all of his physical function as soon as possible and would like to have therapy services at his home if the nurses are not able to provide that skill.
