CARE PLAN INSTRUCTIONS

Complete a care plan for one patient with the following information:

1. One 3 Part NANDA Nursing Diagnosis

2. One short term goal

3. One long term goal

4. Two Interventions for each goal

5. Medication Active Learning Template for one medication that the patient is on related to diagnosis.

Note: The care plan must be on a different diagnosis for each week and the Medication Active Learning Template must be one of the medications that the patient is taking and must be a different medication each week.
