BACKGROUND INFORMATION:

· Include information on person completing this report. Who are they? Age, race/ethnicity.

· Provide info on past family history. Family, where they were raised, occupations of parents, education level of parents, occupations of siblings, education of siblings, any past info on abuse, how family got along.

· Provide info on current family history. Married, single, divorced? Get along with spouse? Does spouse work? Children? How many? Who is responsible for parenting? Any marital difficulties?

· Provide info on education history. How did the client do in school? Regular education? Special education? Peer difficulties? Grades? Problems learning? Failed grades? What about after high school? College? What did they study? How did they perform?

· Provide occupation/employment history. What jobs have they held? How did they perform in the jobs? What is there length of employment? Shortest, longest? Did they enjoy the jobs? Were the jobs difficult for them? If so, why? Where do they currently work? Any work difficulties? Any difficulties getting along with co-workers? Have they ever been fired? If yes, what for?

OBSERVATIONS DURING ASSESSMENT SESSIONS:

Provide information on any observations that occurred during assessment. Look for things such as difficulty understand the information asked of them in the inventories. Unwillingness to provide information during the interviews or completing the inventories. How did they relate with you (the examiner)? Were they angry, cooperative, defensive, guarded? Any language barriers? Were they distracted during the process? The purpose of this section is to provide any information that you observed that may be important to the testing. For example, if the client was not cooperative, how was this demonstrated? This may then indicate that the results of this assessment may not be valid.

CAREER MODELS/THEORIES:

Refer to your textbook by Sharf. You are to select at least four (4) adult models from the book that you have studied thus far. For example, you can select Ginzberg and Associates, Super, Tiedeman and O-Hara, Havighurst, Erikson, Kram, or others. The purpose of this section is for you to explain the model in general terms and then apply it to yourself. If you select i.e. Super’s model, you would explain the theory and the stages. Then you would describe the stages to yourself and where you find yourself, specifically what stage are you at.

CONCLUSIONS:

Provide some background information on the person tested. Provide statements on who client, purpose of testing and some educational/occupational history that may be relevant to this report. This information should be no more than 2 paragraphs long but longer than 3 sentences. Provide an overview of the career theories and then review the tests given. State what was given and what the report said. Do this for all tests given. Then provide a paragraph or more on what this information means about the person and how this can be applied to a career or to what the purpose is of the report. Attempt to tie in the report results with making recommendations for their future career or education. Be creative here. Look at recommendations such as college, vocational training, certain career choices, interning, volunteering, etc.

RECOMMENDATIONS:

Provide at least one career goal which is based on the information your gained from this testing.
