Objective 1: In a 16 week implementation period, implement the nurse-led CAUTI Prevention Bundle on Valley View's Medical-Surgical unit to support the normalization of evidence-based practices for urinary catheter insertion, maintenance, daily need assessment, documentation and timely removal. |

Use weekly observation audits, completed bundle checklists and documentation reviews to measure nursing compliance with the CAUTI Prevention Bundle. Compliance will be determined as the percentage of completed bundle components that are completed in compliance with hospital policy. The purpose of the project is to reach 90% compliance with all of the bundle elements by the end of the implementation period amongst all staff. |

Objective 2: Achieve a 25% reduction in the number of healthcare-associated urinary tract infections (HCAUTIs) among adults with indwelling urinary catheter (IUC) on the medical-surgical unit in a 16-week quality improvement project by implementing the IUC nurse-led CAUTI Prevention Bundle. |

Compare baseline CAUTI surveillance data prior to implementation with monthly infection surveillance report through the hospital's Infection Prevention Department post implementation. The success will be measured by a 25% decrease in CAUTI rates from the baseline rate over the duration of the project. |

Objective 3: Strengthen nursing knowledge and confidence about evidence-based CAUTI prevention practices through standardized education prior to implementing the intervention in regard to: catheter insertion, maintenance, documentation, daily necessity assessments and nurse-driven removal for CAUTI. |

Conduct a pre-education and post-education knowledge quiz to the involved nursing staff. The outcomes of knowledge improvement will be measured by the mean score of quizzes taken before and after the educational session. The goal of the project is for at least 90% of the nurses participating in the project to score 85% or better on the post-test. |

Objective 4: Implement standardized daily catheter reviews as part of the daily nursing routine during the implementation, to increase adherence to daily catheter necessity assessments and timely removal of the catheter. |

Use a standard patient chart audit tool to review patient charts & completed catheter necessity documentation. The percentage of patients receiving documented daily catheter necessity assessment will be used to assess compliance. The objective is to ensure that 95% of the project is documented throughout. |

Objective 5: Assess the attitudes and beliefs of staff towards the usefulness, feasibility, and sustainability of the nurse-led CAUTI Prevention Bundle following implementation to encourage the sustainability of evidence-based catheter care practices. |

Give a quick anonymous staff satisfaction survey after the project is completed, on a 5-point Likert scale. Survey results will be used to evaluate perceived ease of use, confidence in the use of the intervention and willingness to continue use of the bundle. Success will be measured when 85% or more of respondents say that they are satisfied and will implement the CAUTI Prevention Bundle going forward. |
