1. Presenting Problem
The client has been having behavioral problems that started to occur when the clients father remarried. The client’s behavior has been defined as anti-social, anxious and depressed and increased delinquent and aggressive behavior. The client has a history of problems socializing, and low self-confidence, difficult sleeping well and complaints of depression.
2. Social History
The client is a 16-year-old African American male, born in Huntsville, Alabama and that is where he currently resides. He currently lives with his father and stepmother. The client is enrolled in the 10th grade at Columbia High School. He takes regular classes and has not been identified for special education. The client’s father reported that the client earns grades of “C’s”. However, he doesn't always complete his assignments as needed, his attendance is good, but his behavior needs to improve.
His parents divorced when he was 8 years old. After the divorce his mother moved to Louisiana and my client remained in the custody of his father in Huntsville, Alabama. A couple years after the divorce the client’s father remarried and this triggered a behavioral pattern that my client constantly struggles with.
3. Family Dynamics
The client is an only child. He has not had any contact or communication with his nothing since the parents’ divorce. The clients’ mother is in Louisiana. His father has had one live-in girlfriend when the client was 9-10 years old who is now married to the client’s father. The client has a good relationship with his stepmother despite being upset that his mother is not present.
The client father has no incarceration or legal issues.

The client has no mental health history
5. Academic and Intellectual history
The client is in the 10th grade. His grades are typically C’s However, he doesn't always complete his assignments as needed, his attendance is good, but his behavior needs to improve. He has never been identified for special education. The client is a mentee in focus on the future mentorship program in Huntsville, Alabama where he learns academics, professionalism and spirituality in preparation for them to lead their community. The client admits to acting out at school due to him being depressed and missing his biological mother. The client has stated that not having a relationship and not hearing from his mother affect him mentally. The client says that he suffers from being depressed, anti-social, and low self-confidence.
6. Medical History
The client reports no development issues.
