CONSENT FOR ANONYMOUS SURVEY
You are being invited to be a part of the study about ‘Early Intervention for Children with Intellectual Disabilities in Ireland Compared To UK.’ It is a research project that will be conducted by XXXXX of the Institute of Technology Carlow. The project main objective is to understand how Ireland is responding to the issues of intellectual disabilities in early years of children. It will be conducted in several schools providing special education in Ireland. There are no risks that are involved in this study if you choose to participate in the study. There are also no costs associated with the participation process. The information that will be provided in the questionnaire will not be shared with other third parties but will be used for the purposes of the study alone.
The survey questionnaire remains anonymous and hence choosing to participate in this study will not require the participant to write their names, the school they are teaching, or any information that will personally identify them. Thus, there will be no one who will be able to identify who participated in the study. This means that nothing that is provided in the questionnaire will affect the future employment in these institutions of learning.
