Sales Manager/CSM: List the hotel staff contacts here – the sales person and anyone else who worked with this group, including CSM and Catering Manager if there is one.

Beverage: Only use this section if there is a bar or if wine is served. For bar, list whether it is Host or Cash, and include the price of cocktails, beer, wine by the glass, and soft drinks. If it’s a package bar (charged per person), use the per-person price. For wine at the tables, name the brand of wine and the price per bottle. Also include any staff charges, such as bartender and cashier fees.
Menu: Each and every menu item must be listed here. This is the menu that your Banquet Chef will cook, and what your banquet staff will serve. Include soft drinks such as coffee in this section.
Room Set-Up: What set-up style did the planner choose? Add any special furniture (such as buffet tables, lectern, stage) if requested. Add special linen, only if requested. Otherwise standard house linen will be used. Add special centerpieces, only if requested.
Audio Visual: List all A/V equipment (including microphone) and prices.
Special Instructions: Anything that doesn’t belong in the boxes above
Payment: Choose Master account or direct bill or collect payment at end of function. Include deposits received.
