Instructions

Falls are an important nursing quality outcome indicator. For this assignment, you will use Microsoft Excel to analyze an existing data set related to nursing home fall incidents and draw conclusions about how your results could be used to improve nursing practice. 

Note:  Later in the semester, you will be learning more about using data and technology for quality improvement and patient safety. As a foundation for your future learning, the goal of this assignment is to give you practice in analyzing data to provide information and knowledge that can be applied in the clinical practice setting.

For those of you who have experience using Microsoft Excel, the calculation and graphing portion of this assignment will be fairly easy. Others who have never used Excel will benefit from learning some new, basic skills in working with this very useful software.

Use the nursing home fall incident data provided to complete the following assignment:

1. Calculate total falls: Using a formula, show the total number of falls for each block of time. Show these values in the shaded cells in row #123 of the spreadsheet. The formula used to calculate each total should be embedded in each cell.

2. Calculate the total and percentage of falls for each nursing shift: Calculate the total number of falls and percentages of falls for each severity level. Show these values in the shaded cells in rows 2, 3 and 4, columns R & S of the spreadsheet. The formula used to calculate each total should be embedded in each cell.

3. Graph results: Create one graph to effectively display the number of falls for each nursing shift. Be sure to title your graph and include clear and meaningful labels. Display your graph on a new sheet.

4. Calculate the total and percentage of falls for each severity level: Sort the data by severity. Calculate the total number of falls and percentages of falls for each severity level. Show these values in the shaded cells in rows 7 and 8, columns R & S of the spreadsheet. The formula used to calculate each total should be embedded in each cell.

5. Graph Results: Create a different graph to effectively display the percentages of each type of fall. Be sure to title and label your graph appropriately. Display your graph on a new sheet.

6. Identify Patterns: Analyze the data. What patterns or trends can you identify from this data analysis?

7. Use Data to Inform Practice: Based on your analysis, how might you use this data to inform nursing practice to reduce falls in the patient care environment? Are there any patterns or trends that may suggest the need for a quality improvement effort? What additional data would you want to gather and analyze to further assess the situation and target improvement interventions?

Submit two documents:

1. Excel workbook, including the calculations on sheet one and graphs on sheets two and three.

2. A Microsoft Word document with your answers to questions 6 & 7. Your document may be as long as needed to answer the questions thoroughly and succinctly and may include references. There is no need for references for this assignment, but please include a cover sheet per APA guidelines.
