Assignment 5 – Incident Investigation
This is an individual assignment worth 5% - DUE – Friday of Week 13 at 11:59pm.
To complete the assignment, review the case posted in the Assignment 5 Folder on FOL.
Using the template provided in course content, inserting more rows as needed:
1) List at least 5 (five) contributing factors that were present for this incident, indicating whether the factors were human, situational or environmental.
2) Develop an action plan to address each of the factors identified in question 1. Be sure to clearly explain the actions required and include details such as timelines, persons responsible, etc.
This assignment will be graded out of 10, as outlined in the assignment rubric.
