Nursing assignment 3

A current research problem related to advanced nursing practice that is of interest to me is the

prevention of heart failure patients being continuously readmitted to the hospital and seeing

improvement with patient education and follow-up care by nurse practitioners. I believe this is

important because there are many readmissions that can be prevented due to limited outpatient

resources, poor medication adherence, and poorly management self-care which harmfully affects

patient outcomes and increases healthcare costs.
