Assignment 3 topic is "Writing an Incident Report" (500 Words from introduction to conclusion. 500 words exclude the text in the Form (see below), the title, date, names, and subject title in the Report. That is the paper will have 500 words minimum excluding the topic title, your name, instructor's name, date, and addressee, but not more than 600 words.).Your Assignment paper should have the following items.

1. Incident Form (see below). 2. Title, DATE, TO, FROM, SUBJECT. 3. Single space, single  column, Times New Roman, 12 font, MS Word file, no color font. 4. Introduction paragraph. 5. Body of your paper. (3-4 small paragraphs with sub-titles, if needed. The paper may contain picture/image (maximum 2 numbers) 6. Concluding remarks.

Also, consider the following.

· Check the rough draft through MS Word for checking spelling, grammar, etc. and make the correction.

· Assignment submitted with spelling, grammatical error, etc. will receive low points.

For your reference, I have attached a SAMPLE. The Sample is simply a Sample! Your paper (Assignment 3 or A3) must include the Report Form. The Sample has the Form,but you need to change the information in the Form based on your Assignment. DO NOT COPY ANY TEXT, EXCEPT THE FORM FROM THE SAMPLE INTO YOUR ASSIGNMENT. Turn in A3 in MS Word format (in MS Windows platform). To stay on safe side, you should also turn in your assignment in PDF format. The instructor will grade your MS Word file.
