Caries Risk assessment

Please use the CAMBRA form with associated guidelines to answer questions about the patient described below. Please be able to answer these questions in your own words. Cutting and pasting will not be sufficient. Feel free to consult other references for help. Please keep this template when submitting your assignment and complete your answers in another color.

Case A-

These are radiographs and a picture (after SDF placement and a prophylaxis) from a healthy 39 year old woman. She presented to RSDM clinic soon after the pandemic shutdown had caused many people to neglect their treatment and hygiene. Her early visits were emergencies therefore these radiographs were not taken until she was comfortable and ready for restorative visits. As you might imagine her oral hygiene was poor but she was enthusiastic about improving this. Additionally, she was very esthetically driven.

1) You are assigned this patient at this stage in her treatment. Please describe your first clinical visit after you have reviewed the medical history, taken vitals and received a “start check” from the faculty to touch the patient. Be specific in describing the following:

a) Describe the intra-oral examination. Be specific as to how this will be accomplished by you. (10 points)

b) The CAMBRA form (Caries management by risk assessment) is completed during treatment planning (This form eventually gets approved by the Operative group at a subsequent appointment). Please discuss the following here:

1) Caries indicators- include definitions and specifics for this patient (15 points)

2) Risk factors- include definitions and specifics for this patient. (15 points)

3) Determine/ calculate this patient’s risk assessment. (5 points)

4) How often will this patient be seen for recall exams. Why does this differ depending upon risk? (5 points)

5) How often will new radiographs be taken? Why does this differ depending upon risk (5 points)

c) Risk assessment ADA- CDT codes are listed below. Indicate which one will be enterrd into the treatment plan and why. (5 points)

D0601-caries risk assessment and documentation with a finding of low risk

D0602- caries risk assessment and documentation with a finding of moderate risk

D0603- caries risk assessment and documentation with a finding of high risk

d) Please describe your plan for helping your patient lower her risk. Write your patient dialogue here in a patient centered manner. Feel free to discuss nutrition and healthy food choices for oral and overall health. Educate your patient. (40 points)
