TITLE PAGE
Your title page should include the following information:

TABLE OF CONTENTS
This is required in order to properly navigate the document.
· Generate in Word
· Ensure that it reflects all section titles, subtitles and page numbers
· Include a listing of Appendices. These should be labeled, not just numbered, so that if the reader wants to look at a certain Appendix it can easily be found. Appendices must be in the order that they are referred to in the body of the text.
· Note that Appendices are extra documents, detailed, background information. Critical information should be included in the body of the document.
