In January 2016, the Brancati Center received a grant from the Baltimore City Health Department to implement the National Diabetes Prevention Program (DPP) of the Centers for Disease Control in East Baltimore. The Brancati Center has the only fully-recognized DPP in the catchment areas of JHH, JHBMC and has filled that void in DPP capacity. Based on the Brancati Center's experience in East Baltimore, transportation and child care are addressed as social determinants of health that are barriers to participation in the DPP. The patient is seen by the diabetes educator for 2 in-person visits in which the patient receives targeted diabetes self-management education, learns how to use the smart meter (provides real time blood glucose to the HCP) and works on medication adherence strategies. After the second visit patients are transferred to the Endocrinologist supervised Virtual Clinic where they are followed by a diabetes NP who will provide monitoring of real time blood glucose, medication management and diabetes education and support. NP communicates with patient via phone, text or email to make changes in medication and lifestyle recommendations. 700 patients have participated.

The patient is seen by the diabetes educator for 2 in-person visits in which the patient receives targeted diabetes self-management education, learns how to use the smart meter (provides real time blood glucose to the HCP) and works on medication adherence strategies. After the second visit patients are transferred to the Endocrinologist supervised VIrtual Clinic where they are followed by a diabetes NP who will provide monitoring of real time blood glucose, medication management and diabetes education and support. NP communicates with patient via phone, text or email to make changes in medication and lifestyle recommendations. 700 patients have participated.

In January 2016, the Brancati Center received a grant from the Baltimore City Health Department to implement the National Diabetes Prevention Program (DPP) of the Centers for Disease Control in East Baltimore. The Brancati Center has the only fully-recognized DPP in the catchment areas of JHH, JHBMC and has filled that void in DPP capacity. Based on the Brancati Center's experience in East Baltimore, transportation and child care are addressed as social determinants of health that are barriers to participation in the DPP. The Brancati Center works with leaders across the hospitals to identify and address additional barriers for their populations. The Anne Arundel County Department of Health, in collaboration with the coalition, applied for and received a grant from the CDC and MDH in the amount of $175,000 for obesity reduction and prevention initiatives through the Million Hearts Initiative. General Information One of three priorities is Obesity Reduction and Prevention; Obesity Prevention Subcommittee’s Action Plan is on p17. Diabetes is linked to obesity within the plan. Anne Aundel has a comprehensive, evidence-based policy of obesity. In terms of creating a healthy environment, completed actions include: a food environmental map of food deserts inc an assessment of country farmer’s markets; healthy events checklist and policy; infrastructure for safe routes to schools; a Bicycle Advisory Committee. Anne Arundel has completed a survey to locate current program information (an inventory); and further research/SWOT analysis of nutrition participant program challenges. Materials have been created for the purpose of communicating information on local programs. Obesity Programs and events related to obesity prevention were promoted through Healthy Anne Arundel Month, Make Health Happen, and the coalition’s social media presence. An OPS Wellness Program survey was developed, implemented, and analyzed by September 2013.
