I don't want to discourage you, but I have read studies on the percentage of opioid addictions that started on the street (typically heroine) vs. medication perscriptions. These studies are large representative samples. Have you read enough to know if the professionals already have answered this question?

You did give it a title, though based on APA rules it should be shorter.
You do not want to use "non-medical' use, because most folks abusing opioids believe that they are using for medical reasons. Read the posted articles on Moodle--and copy the way they define the construct of opioid abuse.
ALSO. all anonymous surveys must start with the language:

Is very shallow. If you read the articles on opioid crisis, you will discover that some people start with street opioid e.g. heroine....but you do not have that question on your survey!
Add a question of if you ever used an opioid, was the first time an illegal substance e.g. Heroine; or a prescription (yours, a relative's or a friends)? Then break it down.
If prescription was was it for?
