Self Awareness - (25 points - 6 pages MINIMUM)
This section must include the results of the DiSC assessment. You may also include the results of
other assessments as you wish. Number each item 1-9.
The most important portion for this section is a personal assessment of the following areas:
1. Personal Behavioral Style (DISC) - 3 points
2. Assessment of Strengths - 2 points
3. Assessment of Weaknesses - 2points
4. Motivational style - 3points
5. Communication style - 3points
6. Emotional response style - 3 points
7. Decision-making style - 3points
8. Behavior undertension or conflictstyle - 3 points
9. Suggestions for Improving Effectiveness in Relating with Others - 3 points

SECTION 2: (Created after completing the Leadership Scorecard in the Course Documents tab.)
Assessing your Leadership Behavior - (25 points - 6 pages MINIMUM)
This must include the results of the leader’s scorecard which is an assessment of your
leadership skills and abilities. Bold, bullet, or italicize each subheading.
Most importantly, it should include:
1) Leadership Scorecard (use a table format) - 10 points
5 or more evaluators - 4 points 4 evaluators -
3 points
3 evaluators - 2 points
2 evaluators - 1 point
Average of evaluator scores - 2 points Your scores - 2
points
Difference in your scores and evaluators - 2 points
2) Evaluation of Scorecard Results (more detail below)

- How will you pursue improving each leadership skill area?
- Provide thoughts on suggested changesforimproving your behavior and
abilities.
- Whatstrategies could you pursue to make these changes?
- You should discuss at least five leadership skills.
3) Team Developmental Strategies – 5 points
- What do you need to do to be a better team leader?
- How does your team need toimprove?
- How can you motivate othersto perform better or to become more
effective?
- You should discuss at least two team skills.
This section must be detailed and thorough. What steps will you take? What areas will you
focus on? Where will you get the help and assistance you need? Discuss accountability and
feedback. Think about how you will get honest and measurable feedback.
