Patient Information and Significance of the problem

The discussion is about the patient suffering from a significant problem. I had to examine him last night. It was a follow-up and live case that required my attention and implementation from the literature.

The patient to be explored suffered from hypertension and diabetes mellitus. The diabetes was diagnosed five years back, and the patient was advised to take the proper precautionary measures. The good dietary management and nutritional needs were guided, but the patient did not follow them. The results are hypertension and high blood sugar.

The patient was asked in the previous check-up to give a follow-up regularly, but he appeared after three years.

The condition was stable at the time of the recent checkup, but the records of diabetes and hypertension were not up to a point. It has been observed that the patient seems less concerned about following the guidelines, but he wanted to get cured by the medication. The family members were also present with the patients, and they showed a random record of high blood pressure that was aggressive. It indicated that the patient could become even worse in the coming days if care is not taken. The medication was given, and the patient was asked to take care of both these problems.

The patients must follow doctors' instructions regarding the disorders and provide a complete follow-up. It is observed that the patients can only follow up for the diseases when the family is supportive. The problem is relevant to my practice as a baccalaureate-prepared nurse because diabetes and hypertension are common but complex disorders. The nurses must ensure that the patients are properly guided and encouraged to get the medication. These are the issues that can be managed but not adequately cured. A record of diabetes and hypertension should be made throughout the follow-up process, and it is found that the patients usually misunderstand the guidelines and do not follow them. The management of patients will help to learn the treatment process of hypertension and diabetes in the future. I will learn how the patients can interact and how they can be appropriately guided.

Role of leadership and change management in addressing the problem
To help patients, nurses must interact with them regularly.
Patients will benefit from the nurses'
improved leadership and change management skills.
For the nurses to be practical, they must be able to communicate and work together with one another.
Nurse leaders must adhere to four ethical principles to do their jobs effectively.
Some examples of this are social fairness and compassion, autonomy;
informed consent;
and the right to choose.
A nurse's job is to care for patients and help them improve by following the proper protocols.
It is possible to learn about nurses'
leadership styles by analyzing several studies on the subject.
In such urgent situations, leaders are instructed to obtain the patient's informed permission (Paarima, 2022).

For the benefit of patients, effective communication and collaboration are essential.

Patients will be unable to understand treatment if nurses do not communicate effectively (Himmelfarb, 2017).
If the situation is not improving, or if the junior nurses or the patients are not participating, the nurses may decide to take action.
To further improve results, they will need to use change management techniques.
Copy of intervention The intervention is to control diabetes and hypertension through dietary and lifestyle modifications.
The first intervention is to include physical activity daily.
The patients are required to walk for at least thirty minutes.
It is suitable for both patients with hypertension and diabetes.
Exercise at least three times a week is essential for the patients to keep them fit.
Moreover, the walk should be kept separated from the routine work.
The following intervention is to ensure that dietary modification is necessary, so the patients must have low sodium and low sugar diet.
There must be no fat and sugar in the diet, and there should be common salt.
It is assumed that the patients must have plenty of vegetables and fruits in their diet.
Fruits and vegetables are the main ingredients in dietary modifications, and the lifestyle must not be sedentary.
A sedentary lifestyle leads to various issues like cardiac issues and obesity.
So, they must be avoided.

Strategies for communicating and collaborating with the patient, family, or group to improve outcomes associated with the problem Listening is one of the essential strategies that help nurses to communicate and collaborate with patients.
Listening and empathy are the two factors that nurses must consider while dealing with the patient.
The broken communication deteriorates the professional bond between the patients and nurses, so it has become vital for them to listen to and communicate accordingly.
Collaboration is essential for their well-being and health promotion.
Understanding the patient's autonomy has become necessary, which is also done through proper communication.
Teamwork and two-way communication are the other strategies that should not be ignored in the patient-nurse relationship.
It is assumed that the patient should participate in the conversation with the nurse and contribute to discussing the problems with the healthcare organizations (Passarella, 2018).

State board nursing practice standards and organizational or governmental policies associated with healthcare technology, care coordination, and community resources
It is up to the states to set guidelines and policies for hypertension and diabetes.
Put another way;

healthcare organizations help nurses and doctors treat patients according to state-based policies, not local or regional ones. The state-based policies have complied with the WHO, CDC, and other vital health organizations. People with diabetes are encouraged to eat healthily, manage sick days, and receive adequate education from the CDC (Olivari, 2018).
The Centers for Disease Control and Prevention (CDC) has mandated that nurses and physicians construct patient-specific wellness and health plans.

Local hospitals and healthcare groups must also follow the CDC's policies, rules, and regulations.
State-wide processes must be followed—the CDC and WHO play a critical role in developing policies and guidelines for treating hypertension and diabetes.
Nurses must follow important norms and regulations when dealing with diseases and disorders of this nature (Olivari, 2018).
Hypertension and diabetes can be managed in part through the use of nursing practices.
With the support of policies, nursing practices can also be used to manage diseases.

These policies will aid a nurse's ability to ensure that patients have enough facilities and opportunities. If nursing practices improve, there will be an impact on both patients and healthcare organizations'
health.

The proposed intervention will improve the quality of care, enhance patient safety, and reduce costs to the system and individuals.
Diabetic and hypertensive disorders will be managed with dietary and lifestyle changes.
The first step is to incorporate physical activity into your daily routine.
There is a 30-minute time limit for the patients to walk.
It is beneficial to both hypertensive and diabetic people.
At least three times a week, the patients must work out to maintain their health.
In addition, the stroll should be maintained apart from the daily routine of the workplace.
The next step is to ensure that the patients'
diets must be altered to include less sodium and sugar.
All food in the diet must be free of fats, sugars and salt.
Patients are expected to consume various fruits and vegetables as part of their daily diets.
In the dietary changes, fruits and vegetables are the most important components, and it's essential to maintain a healthy lifestyle.
Sedentary lifestyles can cause various health problems, including heart disease and obesity.
Consequently, they must be avoided.

Advances in diabetes and hypertension patient treatment have generated a large body of research.
Medical and healthcare groups have accomplished certain breakthroughs in diabetes and hypertension, according to reports.
Diabetes administration syringes and glucometers are two types of diabetic technology.
Patients are instructed to keep close tabs on their blood glucose levels and to take their diabetes medication as prescribed.
Diabetes telemedicine is another example of cutting-edge technology.

It has been noted that patients with diabetes who are not hospitalized require virtual medical care and management (Khan, 2018).
One of the most significant developments in healthcare is telemedicine, which has enabled doctors to treat diabetes online.
This technology has been shown to help individuals with diabetes and their doctors, who typically consult with each other.
Certain technological advantages, such as blood pressure monitors, come with hypertension.
When it's determined that an illness or disease is chronic, it must be treated accordingly.
Diabetes and hypertension can be better controlled using mobile devices and applications.
The patients have been able to preserve their medical and nutritional well-being thanks to this new technology.

Conclusion It is concluded that the interventions required for the current patient are dietary and lifestyle modifications.
A healthy lifestyle is necessary for the patient to overcome hypertension and diabetes.

The nurses need to contribute to the health and wellness of patients.
It is necessary to have effective communication to understand the queries of patients.
