Assignment Overview:
Though the Root Cause model may seem to be a simple process, problems and their root causes are usually very complex. The purpose of this week’s and week 4’s assignment is to give you opportunities to practice the process and begin to learn how to apply it in real-world situations. There is no single correct answer to the problems and their root causes in these case studies. As long as you provide a logical, viable explanation and support it with evidence from the readings and lectures, you will be successful in these assignments.
Part 1: Case Study 1 – Root Cause Analysis

Directions: Write 4 to 5 paragraphs (600-word minimum) identifying and explaining the following:
· Overarching primary problem
· The root cause for the identified problem
· Questions & information/data that led to discovering the root cause
· Additional information/data you would like to have to find the root cause
· The solution to the problem
