I think the most important role a nurse can have in community health with a family centered care is to provide coordination, education, and care to the patient and sensitivity of who the patient considers family. Teaching the family and keeping them updated to allow them to be on the same page of care for the patient as well as the goals for the patient. Family is the support system often that assist the patient to return to a healthier state, generally with the help of others or "family". 

According to Towards a universal model of family centered care: A scoping review," n.d.Families play an important role meeting the care needs of individuals who require assistance due to illness and/or disability. Yet, without adequate support their own health and wellbeing can be compromised.

An area of difficulty this week has been being transferred multiple times when attempting to contact a patient within a facility, which creates a little frustration for myself, I reminded myself to remain patient, and consistent. 

A new intervention I found was, within the scripts and training books, there is resources in which you can utilize depending on the response you get from your patient, or caregiver. To help address their concerns as well as keep a kind demeanor.

As I continue the  tele-health volunteer program, one challenge I was presented with  was a family member that had lost a loved one and now was experiencing symptoms such as panic attacks and generalized anxiety.  As we talked and conversed on the phone, this family member was able to look back at these health conditions and find a common thread that triggered them and it was when after a long day dealing with work and life's daily grind.  As this family member finally got home and was able to "relax", feelings overwhelmed and gathered.  These were centered on the loss of the loved one.  This family member was still grieving and the grief was being displayed not only on an emotional level, but on a physical level as well.  As I listened and gave her words of comfort over the phone, it was important for me to give her resources in order to successfully find ways to let her grief come to the surface in order for the grieving process continue in a healthy way.
