The major objectives for all health maintenance exams should focus on providing guidance toward health promotion and optimal health, to screen for chronic disease, and to provide age appropriate screenings and immunizations (Heidelbaugh, 2018). For this man during his health maintenance visit his vitals signs, height, and weight should be collected and his BMI calculated (Heidelbaugh, 2018). It is also recommended for men between the ages of 40 and 75 years old to have a lipid screening/lipid panel performed during their health maintenance visit (Heidelbaugh, 2018).
The patient reported having a slightly elevated blood pressure some years ago, he should be screened for hypertension during this visit. Screening for hypertension should start for adults when they are eighteen years old, but for patients forty years old and older should be screened at least annually (Sokol, 2018). Since the man hasn’t seen a physician in four years, the practitioner should complete a Framingham risk score this screening tool provides a 10- year risk for cardiovascular disease (Sokol, 2018). A cardiovascular risk assessment should be completed for patients twenty years old and older every three to five years, the two most commonly used are the Framingham risk score and the American College of Cardiology/American Heart Association’s risk calculator (Sokol, 2018). Since the patient has a family history of heart disease and his symptoms of exertional dyspnea and nocturia are compelling, further cardiac testing should be conducted that is outside the norm for a health maintenance/wellness visit.
The patient should also be screened for HIV regardless if he is sexually active or not (U.S. Department of Health and Human Services, 2019). He should be screened for tobacco use and offered tobacco smoking cessation counseling and services if applicable (U.S. Department of Health and Human Services, 2019). It is recommended that the patient be screened for unhealthy alcohol use and depression as well (Sokol, 2018). If the patient is sexually active a syphilis screening and counseling on sexually transmitted infections should be provided (U.S. Department of Health and Human Services, 2019). A discussion about screening for prostate cancer should be completed since the patient’s maternal uncle was diagnosed with prostate cancer, and especially if his uncle was less than 65 years old when he was diagnosed (Sokol, 2018). Men who are over the age forty are considered having a high risk for prostate cancer if they have one or some of the following risk factors; they are black, have a family history of prostate cancer, especially if prostate cancer occurred in a relative younger than sixty fiver years old, or men who have the BRCA1 or BRCA2 gene mutations or are likely to have these mutations (Sokol, 2018). For men between the ages of 45- years- old and 54- years-old living in the United States in 2015 the leading cause of death was heart disease followed by cancer, unintentional injuries, suicide, chronic liver disease, diabetes, stroke, chronic lower respiratory diseases, homicide, and HIV (Centers for Disease Control and Prevention, 2018).
