The first page contains the information you need to use for the Participants section of the Methods section.  For the Materials sectiion, you just need to describe the survey and what it contained (Demographic items (sex, race, age) and the TIPI scale), In the procedure describe  how participants competed the survey after being shown either a messy room or clean room picture and being told that the occupant was either male or female. 

For the Design section, you can say that the project utilizes a 2 (Occupant Sex) X 2 (Room Condition: Messy vs. Clean) between subjects design with scores on the TIPI scale serving as dependent variables.
