What are the ethical and moral issues that present themselves when communities/cities/states choose to address or not address the health needs of the underserved populations? Choose one of these populations and discuss what resources may be available to them.

The main goal of the public health department is to safeguard the public’s health and by doing that improve community health status in the US, when communities/cities/states find ways to promote the health needs of the underserved population it leads to positive safeguards. Approaching their health needs in an early manner can promote early prevention or diagnosis of a disease on the underserved population leads to reducing the spread of diseases, decrease on morbidity and mortality issues, promoting more health education can decrease medical cost and promoting a healthier community. Negative outcomes such as not meeting the community and underserved population health needs will most likely cause frustration, negative attitude to the health care nurses and add a complicated solution to health care problems in any community.

One of these underserved populations is the lesbian, gay, bisexual, and transgender (LGBT). Nearly 1 in 5 members of the LGBTQ community has avoided seeking medical care because they have fear facing discrimination. There are some resources that are available to address healthcare for the LGBT community, which are the national LGBTQIA healthcare center that provides educational programs and consultation to health organizations. According to Martos, A. J., Wilson, P. A., & Meyer, I. H, ( 2017) study found transgender older adults were at significantly higher risk of poor physical health, disability, depressive symptomatology, and perceived stress, fear of accessing health services, lack of physical activity, internalized stigma, victimization, and lack of social support. There is also the Gay, Lesbian, Transgender, and Bisexual Health Access Project that is a collaborative and community-based program funded by the Massachusetts Department of Public Health which mission is to improve the health, safety, and well-being of lesbian, gay, bisexual, and transgender (LGBT) individuals (Martos, A. J., Wilson, P. A., & Meyer, I. H, 2017). However, the Public health nurses make a significant difference in improving the health of a community by monitoring and assessing critical health status indicators such as immunization level, infant mortality rates, and communicable diseases (Stanhope, M., & Lancaster, J, 2016).

Ethical problems occur when there are difficulties in acting without compromising an individual's ethical principle or when professional, societal, or personal morals are conflicted. The common ethical issues facing the underserved population are moral distress where the service provider knows the right action to take but does not. Ethical conflicts also occur when action fulfills some good morals while compromising others. The other ethical issue is the ethical quandary, which arises on how to possess moral authority in decision-making rather than approaching particular issues.

Eradication of the healthcare disparities between the poor and the rich has been a big ethical challenge facing the world. It is not only medical ethics problem but also social justice and brings up the question on physicians' political and social responsibilities (Gwede, Quinn & Green, 2016). However, the work obligation towards social justice and the healthcare system are not acknowledged universally as professional responsibilities. The corporates interests and the healthcare system works together with the corporate prosperity superseding the benefits patient’s benefits. Health policies, government, societal attitude on poverty and illness, and insurers continue to influence the relationship between the patients and practitioners.

In the case of undeserved patients, healthcare providers and activists should advocate for health care equity in the society to ensure that their patients receive compassionate, ethical and high quality care. Undeserved patients are mostly the cultural and ethnic group members whose values and customs of decision-making differs from those of more privileged members who usually dominate healthcare professions. Additionally, underserved patients mostly have less education, significant health disorders, low health numeracy and literacy (Iheanacho, Jordan & Dike, 2020). Therefore, they require additional assistance to understand their medical situations and actively participate in medical care decisions.

Doctor Huang explains the following when you look at the underserved, “certain urban and rural communities (designated as Health Profession Shortage Areas or Medically Underserved Areas by the federal Health Resources and Services Administration) lack primary medical care, primary care physicians, and other health resources. ...Factors such as transportation difficulties, limited health literacy, lack of fluency in English, low income level, and low education level contribute to the designation of “underserved” (Huang, 2011). How do we best reach out to these populations and affect change?
