Leadership Style Survey
ASSESSING YOUR LEADERSHIP STYLE
INSTRUCTIONS: The following 35 statements describe various aspects of leadership behavior.
Think about a work group that you are currently leading, or a work group that you have led in
the past. It could be at your workplace, or for a school project, or as a leader or coach of some
community group or team. Keep that group in mind, and answer the following questions based
on how you would be likely to act as the leader for that group. (If you have never been a leader,
answer the questions as you imagine you would if you were in a leadership position at your
workplace, school, or community.)
For each item, check whether you would be likely to behave in the described ways
Always (A), Frequently (F), Occasionally (O), Seldom (S), or Never (N).

SCORING INSTRUCTIONS
The scoring for this assessment is complicated, so please follow the instructions carefully.
1. Circle statement numbers 8, 12, 17, 18, 19, 30, 34, and 35. For each of these statements that
you checked S (seldom) or N (never), place a number 1 in the left margin next to the circled
number.
2. For the 27 statements not circled, place a 1 in the left margin next to the statement number
for each statement that you checked A (always) or F (frequently).
3. You should now have 1’s in the left margin of many of the 35 statements.
4. Circle the 1’s in the margin you have written in front of the following statements:
3, 5, 8, 10, 15, 18, 19, 22, 24, 26, 28, 30, 32, 34, and 35. Count the number of circled 1’s you
have and write it here: R = __________. This is your relationship-oriented leadership score.
5. Count the number of 1’s you have that are not circled for the remaining statements,
and write that number here: T = __________. This is your task-oriented leadership score.
INTERPRETING YOUR SCORE Complete this section after you have read about the Hersey-Blanchard Model of leadership (Chapter 10.4 in your textbook)
Which of the following best characterizes your scores on this assessment?
